Со стороны органа зрения: редко — нарушение зрения/нечеткость зрения.
Со стороны ЖКТ: нечасто — диарея, тошнота/рвота, вздутие живота и метеоризм, запор, сухость слизистой оболочки полости рта, боль и дискомфорт в животе.
Со стороны печени и желчевыводящих путей: нечасто — повышение активности печеночных ферментов (трансаминаз, ГГТ); редко — повышение концентрации билирубина; частота неизвестна — гепатоцеллюлярные повреждения, желтуха, гепатоцеллюлярная недостаточность.
Со стороны кожи и подкожных тканей: нечасто — кожная сыпь/экзантема/высыпание, кожный зуд; редко — крапивница, ангионевротический отек; частота неизвестна — синдром Стивенса-Джонсона, синдром Лайелла, мультиформная эритема, светочувствительность, подострая кожная красная волчанка (ПККВ).
Со стороны скелетно-мышечной системы и соединительной ткани: нечасто — перелом шейки бедренной кости, костей запястья или позвоночника; редко — артралгия, миалгия; частота неизвестна — мышечный спазм как следствие нарушения электролитного баланса.
Со стороны почек и мочевыводящих путей: частота неизвестна — интерстициальный нефрит (с возможным прогрессированием до почечной недостаточности).
Со стороны половых органов и молочной железы: редко — гинекомастия.
Общие расстройства и нарушения в месте введения: нечасто — астения, чрезмерная утомляемость и недомогание; редко — повышение температуры тела, периферический отек.

При применении пантопразола, лиофилизат для приготовления раствора для в/в введения, в соответствии с показаниями и в рекомендованных дозах побочные эффекты возникают крайне редко. Наиболее частой нежелательной побочной реакцией является тромбофлебит в месте введения препарата. Диарея и головная боль наблюдаются примерно у 1% пациентов.

Generic Name: Pantoprazole
Product Name: Somac Tablets (20mg or 40mg on Prescription)
Indication
Somac tablets are used to treat the following symptoms and conditions in adults:
- Relieving symptoms and healing gastrointestinal wounds caused by excessive gastric acid productionassociated with:
- Duodenal ulcer;
- Gastric ulcer;
- Reflux oesophagitis;
- Gastrointestinal lesions which have been treated with H2 receptor antagonist medication, but which have failed to heal;
- Zollinger-Ellison syndrome.
- Stopping the return of symptoms of reflux oesophagitis in individuals who have previously had moderate–severe symptoms of reflux, but have been successfully treated;
- Eradication ofthe bacteria Helicobacter pylori. In these cases, antibiotics should also be taken;
- Relief of symptoms of gastro-oesophageal reflux disease (GORD);
- Prevention of gastrointestinal lesions and dyspepsia caused by long term treatment with non-selective non-steroidal anti-inflammatory drugs (NSAIDs).
Somac tablets can also be used to treat children older than five years of age, but should not be used for more than 8 weeks. For children, Somac tablets are used to treat:
- Symptomatic GORD; and
- Reflux oesophagitis.
Somac is available in several different forms. For more information on other available forms, see Somac Granules (40mg on Prescription), Somac Injection and Somac Heartburn Relief (Over-the-Counter).
Action
The active ingredient in Somac 40 mg tablets is pantoprazole, which belongs to a group of medications known as proton pump inhibitors. These medications control an enzyme called H+/K+-ATPase which produces gastric acid in the parietal cells of the stomach. By controlling this enzyme, pantoprazole decreases the amount of acid produced by the stomach. When acid production is controlled, symptoms like acid reflux and stomach lesions caused by excessive acid production are also controlled.

Dose information
The doctor who prescribed Somac tablets will tell you how much medication to take and how often.
Somac tablets should be swallowed whole. They should not be crushed or chewed.
Contraindications
Somac tablets should not be taken in certain situations. Your doctor will discuss alternatives that will be more suitable for you. Do not take Somac tablets if you have:
- Allergies to any of the medication’s components;
- Cirrhosis;
- Moderate to severe liver failure;
- Moderate to severe kidney failure;
- Are currently taking atazanavir.
Precautions
Some circumstances need special consideration before taking Somac tablets. Tell your doctor if you have any of the following:
- Significant unintentional weight loss;
- Recurrent vomiting;
- Difficulty swallowing;
- Vomiting blood;
- Anaemia;
- Black ‘tarry’ stools;
- Suspected or confirmed gastric ulcer;
- Cyanocobalamin (vitamin B12) deficiency or risk of deficiency.
Use in children
The use of Somac tablets in children under five years of age has not been studied. Do not give Somac tablets to a child under five years of age.
Use in pregnancy
Somac tablets are a Pregnancy Category B3 medication. The safety of use during pregnancy has not been established. Somac tablets should not be used if you are pregnant or planning to become pregnant.
Use while breastfeeding
Somac tablets should not be used if you are breastfeeding or planning to breastfeed.
Schedule
Somac tablets are Schedule 4 – prescription only medication.1
Common side effects
All medicines have side effects. Most commonly the side effects are minor; however, some can be more serious. Usually the benefits of taking a medication outweigh the associated side effects. Your doctor would have considered these side effects before starting you on Somac tablets.
Common side effects are those which occur in more than 1% of patients given Somac tablets. These include:
- Fatigue;
- Asthenia;
- Increased sweating;
- Headache;
- Diarrhoea;
- Severe belching;
- Constipation or flatulence;
- Dry mouth;
- Upper abdominal pain;
- Metallic taste.
Uncommon side effects
Side effects which occur in less than 1% of patients given Somac tablets are considered uncommon. Patients do not necessarily experience any of these side effects, so do not become alarmed by this list:
- Dizziness;
- Nausea and vomiting;
- Allergic skin reactions, including pruritus and skin rash;
- Blurred vision.
Rare side effects occur in less than 0.1% of patients given Somac tablets. These include:
- Fever;
- Anaphylactic reactions including anaphylactic shock and peripheral oedema;
- Hypertension;
- Rectal disorder;
- Colonic polyp;
- Joint pain;
- Psychiatric disorders, including depression, hallucination, disorientation and confusion. These conditions are particularly likely in patients who have previously experienced psychiatric disorders, and those who are already suffering from a psychiatric disorder may find that their symptoms become worse. Psychiatric disorders are also more common in patients who have an increased risk for these conditions (e.g. patients who have direct relatives who have a psychiatric condition);
- Anaemia;
- Sepsis.
Very rare side effects occur in less than 0.01% of people given Somac tablets. They include:
- Chest pain;
- Hot flushes;
- Circulatory collapse;
- Reduced movement;
- Speech disorder;
- Faecal discolouration;
- Increased saliva;
- Tinnitus;
- Increased liver enzymes;
- Hepatic failure;
- Cholestatic hepatitis;
- Bilirubinaemia;
- Jaundice;
- Hypertriglyceridaemia;
- Muscle pain;
- Pain, including skeletal pain;
- Interstitial nephritis;
- Thrombocytopaenia;
- Increased bleeding;
- Anxiety;
- Leukopaenia;
- Breathlessness;
- Angioedema;
- Urticaria;
- Severe skin reactions, including Stevens-Johnson syndrome, toxic epidermal necrolysis, erythema multiforme, Lyell’s syndrome and photosensitivity;
- Changes to sense of smell and taste;
- Flushing;
- Conjunctivitis.

PRECAUTIONS
Presence Of Gastric Malignancy
In adults, symptomatic response to therapy with PROTONIX does not preclude the presence of gastric malignancy. Consider additional follow-up and diagnostic testing in adult patients who have a suboptimal response or an early symptomatic relapse after completing treatment with a PPI. In older patients, also consider an endoscopy.
Acute Interstitial Nephritis
Acute interstitial nephritis has been observed in patients taking PPIs including PROTONIX. Acute interstitial nephritis may occur at any point during PPI therapy and is generally attributed to an idiopathic hypersensitivity reaction. Discontinue PROTONIX if acute interstitial nephritis develops.
Clostridium Difficile-Associated Diarrhea
Published observational studies suggest that PPI therapy like PROTONIX may be associated with an increased risk of Clostridium difficile associated diarrhea, especially in hospitalized patients. This diagnosis should be considered for diarrhea that does not improve.
Patients should use the lowest dose and shortest duration of PPI therapy appropriate to the condition being treated.
Bone Fracture
Several published observational studies suggest that PPI therapy may be associated with an increased risk for osteoporosis-related fractures of the hip, wrist, or spine. The risk of fracture was increased in patients who received high-dose, defined as multiple daily doses, and longterm PPI therapy (a year or longer). Patients should use the lowest dose and shortest duration of PPI therapy appropriate to the condition being treated. Patients at risk for osteoporosis-related fractures should be managed according to established treatment guidelines.
Cutaneous And Systemic Lupus Erythematosus
Cutaneous lupus erythematosus (CLE) and systemic lupus erythematosus (SLE) have been reported in patients taking PPIs, including pantoprazole sodium. These events have occurred as both new onset and an exacerbation of existing autoimmune disease. The majority of PPIinduced lupus erythematous cases were CLE.
The most common form of CLE reported in patients treated with PPIs was subacute CLE (SCLE) and occurred within weeks to years after continuous drug therapy in patients ranging from infants to the elderly. Generally, histological findings were observed without organ involvement.
Systemic lupus erythematosus (SLE) is less commonly reported than CLE in patients receiving PPIs. PPI associated SLE is usually milder than non-drug induced SLE. Onset of SLE typically occurred within days to years after initiating treatment primarily in patients ranging from young adults to the elderly. The majority of patients presented with rash; however, arthralgia and cytopenia were also reported.
Avoid administration of PPIs for longer than medically indicated. If signs or symptoms consistent with CLE or SLE are noted in patients receiving PROTONIX, discontinue the drug and refer the patient to the appropriate specialist for evaluation. Most patients improve with discontinuation of the PPI alone in 4 to 12 weeks. Serological testing (e.g. ANA) may be positive and elevated serological test results may take longer to resolve than clinical manifestations.
Cyanocobalamin (Vitamin B-12) Deficiency
Generally, daily treatment with any acid-suppressing medications over a long period of time (e.g., longer than 3 years) may lead to malabsorption of cyanocobalamin (Vitamin B-12) caused by hypo- or achlorhydria. Rare reports of cyanocobalamin deficiency occurring with acid-suppressing therapy have been reported in the literature. This diagnosis should be considered if clinical symptoms consistent with cyanocobalamin deficiency are observed.
Hypomagnesemia
Hypomagnesemia, symptomatic and asymptomatic, has been reported rarely in patients treated with PPIs for at least three months, in most cases after a year of therapy. Serious adverse events include tetany, arrhythmias, and seizures. In most patients, treatment of hypomagnesemia required magnesium replacement and discontinuation of the PPI.
For patients expected to be on prolonged treatment or who take PPIs with medications such as digoxin or drugs that may cause hypomagnesemia (e.g., diuretics), health care professionals may consider monitoring magnesium levels prior to initiation of PPI treatment and periodically.
Tumorigenicity
Due to the chronic nature of GERD, there may be a potential for prolonged administration of PROTONIX. In long-term rodent studies, pantoprazole was carcinogenic and caused rare types of gastrointestinal tumors. The relevance of these findings to tumor development in humans is unknown.
Interference With Urine Screen For THC
There have been reports of false positive urine screening tests for tetrahydrocannabinol (THC) in patients receiving PPIs, including PROTONIX.
Concomitant Use Of PROTONIX With Methotrexate
Literature suggests that concomitant use of PPIs with methotrexate (primarily at high dose; see methotrexate prescribing information) may elevate and prolong serum levels of methotrexate and/or its metabolite, possibly leading to methotrexate toxicities. In high-dose methotrexate administration, a temporary withdrawal of the PPI may be considered in some patients .
Patient Counseling Information
Advise the patient to read the FDA-approved patient labeling (Medication Guide).

Drug Interactions
Instruct patients to inform their healthcare provider of any other medications they are currently taking, including rilpivirine-containing products , high dose methotrexate and over-the-counter medications.
Pregnancy
Inform female patients of reproductive potential that PROTONIX may cause fetal harm and to inform their prescriber of a known or suspected pregnancy.
Administration
- Caution patients that PROTONIX For Delayed-Release Oral Suspension and PROTONIX Delayed-Release Tablets should not be split, crushed, or chewed.
- PROTONIX oral suspension packet is a fixed dose and cannot be divided to make a smaller dose.
- Tell patients that PROTONIX Delayed-Release Tablets should be swallowed whole, with or without food in the stomach.
- Let patients know that concomitant administration of antacids does not affect the absorption of PROTONIX Delayed-Release Tablets.
- Advise patients to take PROTONIX For Delayed-Release Oral Suspension approximately 30 minutes before a meal.
- Advise patients that PROTONIX For Delayed-Release Oral Suspension should only be administered in apple juice or applesauce, not in water, other liquids, or foods.
Nonclinical Toxicology
Carcinogenesis, Mutagenesis, Impairment Of Fertility
In a 24-month carcinogenicity study, Sprague-Dawley rats were treated orally with pantoprazole doses of 0.5 to 200 mg/kg/day, about 0.1 to 40 times the exposure on a body surface area basis of a 50 kg person dosed with 40 mg/day. In the gastric fundus, treatment at 0.5 to 200 mg/kg/day produced enterochromaffin-like (ECL) cell hyperplasia and benign and malignant neuroendocrine cell tumors in a dose-related manner. In the forestomach, treatment with 50 and 200 mg/kg/day (about 10 and 40 times the recommended human dose on a body surface area basis) produced benign squamous cell papillomas and malignant squamous cell carcinomas. Rare gastrointestinal tumors associated with pantoprazole treatment included an adenocarcinoma of the duodenum with 50 mg/kg/day and benign polyps and adenocarcinomas of the gastric fundus with 200 mg/kg/day. In the liver, treatment at 0.5 to 200 mg/kg/day produced dose-related increases in the incidences of hepatocellular adenomas and carcinomas. In the thyroid gland, treatment with 200 mg/kg/day produced increased incidences of follicular cell adenomas and carcinomas for both male and female rats.
In a 24-month carcinogenicity study, Fischer 344 rats were treated orally with doses of 5 to 50 mg/kg/day of pantoprazole, approximately 1 to 10 times the recommended human dose based on body surface area. In the gastric fundus, treatment with 5 to 50 mg/kg/day produced enterochromaffin-like (ECL) cell hyperplasia and benign and malignant neuroendocrine cell tumors. Dose selection for this study may not have been adequate to comprehensively evaluate the carcinogenic potential of pantoprazole.
In a 24-month carcinogenicity study, B6C3F1 mice were treated orally with doses of 5 to 150 mg/kg/day of pantoprazole, 0.5 to 15 times the recommended human dose based on body surface area. In the liver, treatment with 150 mg/kg/day produced increased incidences of hepatocellular adenomas and carcinomas in female mice. Treatment with 5 to 150 mg/kg/day also produced gastric-fundic ECL cell hyperplasia.
A 26-week p53 +/- transgenic mouse carcinogenicity study was not positive.
Pantoprazole was positive in the in vitro human lymphocyte chromosomal aberration assays, in one of two mouse micronucleus tests for clastogenic effects, and in the in vitro Chinese hamster ovarian cell/HGPRT forward mutation assay for mutagenic effects. Equivocal results were observed in the in vivo rat liver DNA covalent binding assay. Pantoprazole was negative in the in vitro Ames mutation assay, the in vitro unscheduled DNA synthesis (UDS) assay with rat hepatocytes, the in vitro AS52/GPT mammalian cell-forward gene mutation assay, the in vitro thymidine kinase mutation test with mouse lymphoma L5178Y cells, and the in vivo rat bone marrow cell chromosomal aberration assay.
There were no effects on fertility or reproductive performance when pantoprazole was given at oral doses up to 500 mg/kg/day in male rats (98 times the recommended human dose based on body surface area) and 450 mg/kg/day in female rats (88 times the recommended human dose based on body surface area).
Use In Specific Populations
Pregnancy
Teratogenic Effects
Pregnancy Category C
Reproduction studies have been performed in rats at oral pantoprazole doses up to 450 mg/kg/day (about 88 times the recommended human dose based on body surface area) and in rabbits at oral doses up to 40 mg/kg/day (about 16 times the recommended human dose based on body surface area) with administration of pantoprazole sodium during organogenesis in pregnant animals. The studies have revealed no evidence of impaired fertility or harm to the fetus due to pantoprazole.
A pre- and postnatal development toxicity study in rats with additional endpoints to evaluate the effect on bone development was performed with pantoprazole sodium. Oral pantoprazole doses of 5, 15, and 30 mg/kg/day (approximately 1, 3, and 6 times the human dose of 40 mg/day on a body surface area basis) were administered to pregnant females from gestation day (GD) 6 through lactation day (LD) 21. On postnatal day (PND 4) through PND 21, the pups were administered oral doses at 5, 15, and 30 mg/kg/day (approximately 1, 2.3, and 3.2 times the exposure (AUC) in humans at a dose of 40 mg). There were no drug-related findings in maternal animals. During the preweaning dosing phase (PND 4 to 21) of the pups, there were increased mortality and/or moribundity and decreased body weight and body weight gain at 5 mg/kg/day (approximately equal exposures (AUC) in humans receiving the 40 mg dose) and higher doses. On PND 21, decreased mean femur length and weight and changes in femur bone mass and geometry were observed in the offspring at 5 mg/kg/day (approximately equal exposures (AUC) in humans at the 40 mg dose) and higher doses. The femur findings included lower total area, bone mineral content and density, periosteal and endosteal circumference, and cross-sectional moment of inertia. There were no microscopic changes in the distal femur, proximal tibia, or stifle joints. Changes in bone parameters were partially reversible following a recovery period, with findings on PND 70 limited to lower femur metaphysis cortical/subcortical bone mineral density in female pups at 5 mg/kg/day (approximately equal exposures (AUC) in humans at the 40 mg dose) and higher doses.
There are no adequate and well-controlled studies in pregnant women. Advise pregnant women of the potential risk of fetal harm. Because animal reproduction studies are not always predictive of human response, this drug should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Nursing Mothers
Pantoprazole and its metabolites are excreted in the milk of rats. Pantoprazole excretion in human milk has been detected in a study of a single nursing mother after a single 40 mg oral dose of pantoprazole sodium. The clinical relevance of this finding is not known. Many drugs which are excreted in human milk have a potential for serious adverse reactions in nursing infants. Based on the potential for tumorigenicity shown for pantoprazole sodium in rodent carcinogenicity studies, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the benefit of the drug to the mother.
Pediatric Use
The safety and effectiveness of PROTONIX for short-term treatment (up to eight weeks) of erosive esophagitis (EE) associated with GERD have been established in pediatric patients 1 year through 16 years of age. Effectiveness for EE has not been demonstrated in patients less than 1 year of age. In addition, for patients less than 5 years of age, there is no appropriate dosage strength in an age-appropriate formulation available. Therefore, PROTONIX is indicated for the short-term treatment of EE associated with GERD for patients 5 years and older. The safety and effectiveness of PROTONIX for pediatric uses other than EE have not been established.
1 Year Through 16 Years Of Age
Use of PROTONIX in pediatric patients 1 year through 16 years of age for short-term treatment (up to eight weeks) of EE associated with GERD is supported by: a) extrapolation of results from adequate and well-controlled studies that supported the approval of PROTONIX for treatment of EE associated with GERD in adults, and b) safety, effectiveness, and pharmacokinetic studies performed in pediatric patients.
Safety of PROTONIX in the treatment of EE associated with GERD in pediatric patients 1 through 16 years of age was evaluated in three multicenter, randomized, double-blind, parallel-treatment studies, involving 249 pediatric patients, including 8 with EE (4 patients ages 1 year to 5 years and 4 patients 5 years to 11 years). The children ages 1 year to 5 years with endoscopically diagnosed EE (defined as an endoscopic Hetzel-Dent score ≥ 2) were treated once daily for 8 weeks with one of two dose levels of PROTONIX (approximating 0.6 mg/kg or 1.2 mg/kg). All 4 of these patients with EE were healed (Hetzel-Dent score of 0 or 1) at 8 weeks. Because EE is uncommon in the pediatric population, predominantly pediatric patients with endoscopically-proven or symptomatic GERD were also included in these studies.
Patients were treated with a range of doses of PROTONIX once daily for 8 weeks. For safety findings see ADVERSE REACTIONS. Because these pediatric trials had no placebo, active comparator, or evidence of a dose response, the trials were inconclusive regarding the clinical benefit of PROTONIX for symptomatic GERD in the pediatric population. The effectiveness of PROTONIX for treating symptomatic GERD in pediatric patients has not been established.
Although the data from the clinical trials support use of PROTONIX for the short-term treatment of EE associated with GERD in pediatric patients 1 year through 5 years, there is no commercially available dosage formulation appropriate for patients less than 5 years of age.
In a population pharmacokinetic analysis, clearance values in the children 1 to 5 years old with endoscopically proven GERD had a median value of 2.4 L/h. Following a 1.2 mg/kg equivalent dose (15 mg for. 12.5 kg and 20 mg for > 12.5 to < 25 kg), the plasma concentrations of pantoprazole were highly variable and the median time to peak plasma concentration was 3 to 6 hours. The estimated AUC for patients 1 to 5 years old was 37% higher than for adults receiving a single 40 mg tablet, with a geometric mean AUC value of 6.8 μg•hr/mL.
Neonates To Less Than One Year Of Age
PROTONIX was not found to be effective in a multicenter, randomized, double-blind, placebo-controlled, treatment-withdrawal study of 129 pediatric patients 1 through 11 months of age. Patients were enrolled if they had symptomatic GERD based on medical history and had not responded to non-pharmacologic interventions for GERD for two weeks. Patients received PROTONIX daily for four weeks in an open-label phase, then patients were randomized in equal proportion to receive PROTONIX treatment or placebo for the subsequent four weeks in a double-blind manner. Efficacy was assessed by observing the time from randomization to study discontinuation due to symptom worsening during the four-week treatment-withdrawal phase. There was no statistically significant difference between PROTONIX and placebo in the rate of discontinuation.
In this trial, the adverse reactions that were reported more commonly (difference of ≥ 4%) in the treated population compared to the placebo population were elevated CK, otitis media, rhinitis, and laryngitis.
In a population pharmacokinetic analysis, the systemic exposure was higher in patients less than 1 year of age with GERD compared to adults who received a single 40 mg dose (geometric mean AUC was 103% higher in preterm infants and neonates receiving single dose of 2.5 mg of PROTONIX, and 23% higher in infants 1 through 11 months of age receiving a single dose of approximately 1.2 mg/kg). In these patients, the apparent clearance (CL/F) increased with age (median clearance: 0.6 L/hr, range: 0.03 to 3.2 L/hr).
These doses resulted in pharmacodynamic effects on gastric but not esophageal pH. Following once daily dosing of 2.5 mg of PROTONIX in preterm infants and neonates, there was an increase in the mean gastric pH (from 4.3 at baseline to 5.2 at steady-state) and in the mean % time that gastric pH was > 4 (from 60% at baseline to 80% at steady-state). Following once daily dosing of approximately 1.2 mg/kg of PROTONIX in infants 1 through 11 months of age, there was an increase in the mean gastric pH (from 3.1 at baseline to 4.2 at steady-state) and in the mean % time that gastric pH was > 4 (from 32% at baseline to 60% at steady-state). However, no significant changes were observed in mean intraesophageal pH or % time that esophageal pH was < 4 in either age group.
Because PROTONIX was not shown to be effective in the randomized, placebo-controlled study in this age group, the use of PROTONIX for treatment of symptomatic GERD in infants less than 1 year of age is not indicated.
Animal Toxicity Data
In a pre- and post-natal development study in rats, the pups were administered oral doses of pantoprazole at 5, 15, and 30 mg/kg/day (approximately 1, 2.3, and 3.2 times the exposure (AUC) in children aged 6 to 11 years at a dose of 40 mg) on postnatal day (PND 4) through PND 21, in addition to lactational exposure through milk. On PND 21, decreased mean femur length and weight and changes in femur bone mass and geometry were observed in the offspring at 5 mg/kg/day (approximately equal exposures (AUC) in children aged 6 to 11 years at the 40 mg dose) and higher doses. Changes in bone parameters were partially reversible following a recovery period.
In neonatal/juvenile animals (rats and dogs) toxicities were similar to those observed in adult animals, including gastric alterations, decreases in red cell mass, increases in lipids, enzyme induction and hepatocellular hypertrophy. An increased incidence of eosinophilic chief cells in adult and neonatal/juvenile rats, and atrophy of chief cells in adult rats and in neonatal/juvenile dogs, was observed in the fundic mucosa of stomachs in repeated-dose studies. Full to partial recovery of these effects were noted in animals of both age groups following a recovery period.
Geriatric Use
In short-term US clinical trials, erosive esophagitis healing rates in the 107 elderly patients (≥ 65 years old) treated with PROTONIX were similar to those found in patients under the age of 65. The incidence rates of adverse reactions and laboratory abnormalities in patients aged 65 years and older were similar to those associated with patients younger than 65 years of age.

Adults: 1. What Somac is used for
Ulcers
Somac is used to treat and help heal duodenal and gastric ulcers.
Depending on the position of the ulcer it is called a gastric or duodenal ulcer. A
gastric ulcer occurs in the stomach. A duodenal ulcer occurs in the duodenum which
is the tube leading out of the stomach.
These can be caused in part by too much acid being made in the stomach.
Most people who have a peptic ulcer also have bacteria called Helicobacter pylori
in their stomach. When Somac is taken with antibiotics the combination therapy will
kill the Helicobacter pylori and let your ulcer heal.
Somac may also be used to prevent ulcers associated with the use of non-steroidal
anti-inflammatory drugs (NSAIDs). These are medicines used to relieve pain, swelling
and other symptoms of inflammation, including arthritis (inflammation of the joints).
Reflux disease
Somac is also used to treat reflux oesophagitis or reflux disease. This can be caused
by “washing back” (reflux) of food and acid from the stomach into the food pipe, also
known as the oesophagus.
Reflux can cause a burning sensation in the chest rising up to the throat, also known
as heartburn.
Somac is also used to prevent reflux oesophagitis from coming back.
Zollinger-Ellison syndrome
Somac is also used to treat a rare condition called Zollinger-Ellison syndrome, where
the stomach produces very large amounts of acid, much more than in ulcers and reflux
disease.
This medicine belongs to a group of medicines called proton pump inhibitors (PPIs).
This medicine works by decreasing the amount of acid the stomach makes, to give relief
from the symptoms and allow healing to take place.
Ask your doctor if you have any questions about why it has been prescribed for you.
Your doctor may have prescribed it for another purpose.
This medicine is not addictive. This medicine is available only with a doctor’s prescription.
This medicine is not expected to affect your ability to drive a car or operate machinery.
However, do not drive a car or operate machines if you experience side effects such
as dizziness or blurred vision.
Somac should not be given to children under 5 years of age.
There is not enough information to recommend the use of this medicine for children
under the age of 5 years.
Before you take Somac
When you must not take it
Do not take Somac if you have an allergy to:
any medicine containing pantoprazole
any of the ingredients listed at the end of this leaflet
Some symptoms of an allergic reaction include skin rash, itching, shortness of breath
or swelling of the face, lips or tongue, which may cause difficulty in swallowing
or breathing.
Do not take this medicine if you have severe liver disease or cirrhosis.
Do not take Somac in combination with antibiotics or any other medicine if:
you are allergic to any of the antibiotics or medicines your doctor may prescribe
with Somac
you have moderate to severe liver or kidney disease
Do not take Somac in combination with atazanavir or nelfinavir (anti-viral medications).
Do not take it after the expiry date printed on the pack or if the packaging is damaged
or shows signs of tampering.
If it has expired or is damaged return it to your pharmacist for disposal.
Before you start to take it
Tell your doctor or pharmacist if you have any allergies to any other medicines, foods,
preservatives or dyes.
Tell your doctor if you have or have had any other medical conditions.
Tell your doctor if you are pregnant, intend to become pregnant, or are breast-feeding.
Your doctor can discuss the risks and benefits involved. If you have not told your
doctor about any of the above, tell them before you take Somac.
Tell your doctor if you have any of the following symptoms:
unintentional weight loss
repeated vomiting
vomiting blood
difficulty or pain when swallowing
you look pale and feel weak
you notice blood in your stools
Your doctor may need to perform some additional tests before you take Somac.
Taking other medicines
Tell your doctor if you are taking any other medicines, including any that you buy
without a prescription from your pharmacy, supermarket or health food shop.
Some medicines and Somac may interfere with each other. These include:
warfarin, phenprocoumon — medicines used to prevent blood clots (anticoagulants)
atazanavir, nelfinavir – medicines used to treat viral infections such as HIV
ketoconazole, itraconazole, posaconazole — medicines used to treat fungal infection
methotrexate — a medicine used to treat arthritis and some types of cancer
erlotinib or related medicines used to treat cancer
tacrolimus, mycophenolate mofetil – medicines used to suppress the immune system
fluvoxamine — a medicine used to treat anxiety and depression
These medicines may be affected by Somac, or may affect how well it works. You may
need to use different amounts of your medicine, or take different medicines.
Your doctor or pharmacist has more information on medicines to be careful with or
to avoid while taking Somac.
How to take Somac
Follow all directions given to you by your doctor or pharmacist carefully.
They may differ from the information contained in this leaflet.
If you do not understand the instructions on the box/bottle, ask your doctor or pharmacist
for help.
How much to take
The usual dose is 40 mg per day.
However, if your doctor also prescribes antibiotics in combination with Somac for
the treatment of duodenal ulcers, the dose of Somac is 80 mg per day. The first 40
mg dose should be taken in the morning and the second should be taken before the evening
meal for 7 days.
Your doctor will prescribe the dose that is right for you.
In children over 5 years of age, the dose of Somac for reflux oesophagitis or reflux
disease is based on weight and may be 20 mg or 40 mg, depending on the condition being
treated. Somac should not be taken for longer than 8 weeks.
The dose and frequency of Somac that your doctor prescribes for you depends on your
medical condition.
Your doctor may change the dose as your condition changes.
How to take it
Swallow your tablets whole with a little water with or without food.
If you are using Somac granules, take it half an hour before a meal without chewing
or crushing the granules. Take the granules in apple juice, orange juice or water
as follows:
1. open sachet
2. sprinkle intact granules into a small volume of apple juice, pulp-free orange juice,
or water (at least 15 mL). Alternatively, a tablespoon may be used
3. mix briefly just before drinking
4. drink straight away
5. to ensure that you have taken all the medicine, rinse the container thoroughly once
or twice with apple juice, orange juice or water to remove any remaining granules
and drink immediately
You can also take the granules in applesauce as follows:
1. open sachet
2. sprinkle intact granules on a teaspoon of applesauce.
3. swallow within 10 minutes of preparation
Somac granules are intended for people who have difficulty swallowing tablets including
those who require a nasogastric tube.
When to take it
Take Somac at about the same time each day.
Taking it at the same time each day will have the best effect. It will also help you
remember when to take it.
If you are taking other medicines, like antibiotics, in combination with Somac therapy,
follow the instructions for the use of each medicine carefully.
Do not crush or chew the tablets or granules. Somac tablets and granules have a special
coating to protect them from the acidic contents of your stomach. For Somac to work
effectively, this coating must not be broken.
How long to take it
Continue taking your medicine for as long as your doctor or pharmacist tells you.
If you forget to take it
If it is almost time for your next dose, skip the dose you missed and take your next
dose when you are meant to.
Otherwise, take it as soon as you remember, and then go back to taking it as you would
normally.
Do not take a double dose to make up for the dose that you missed.
This may increase the chance of getting an unwanted side effect.
If you are not sure what to do, ask your doctor or pharmacist.
If you have trouble remembering when to take your medicine, ask your pharmacist for
hints.
If you take too much (overdose)
Immediately telephone your doctor or Poisons Information Centre (telephone Australia
13 11 26 or New Zealand 0800 764 766), or go to Accident and Emergency at your nearest
hospital, if you or anyone else may have taken too much Somac. Do this even if there
are no signs of discomfort or poisoning.
Urgent medical attention may be needed.
While you are taking Somac
Things you must do
If you are about to be started on any new medicine, remind your doctor and pharmacist
that you are taking Somac.
Tell any other doctors, dentists and pharmacists who treat you that you are taking
this medicine.
If you are going to have surgery, tell the surgeon that you are taking this medicine.
If you become pregnant while you are taking this medicine, tell your doctor or pharmacist
immediately.
If you are about to have any blood tests, tell your doctor that you are taking this
medicine.
It may interfere with the results of some tests.
Keep all of your doctor’s appointments so that your progress can be checked.
Your doctor may do some tests from time to time to make sure the medicine is working
and to prevent unwanted side effects.
Tell your doctor if your reflux symptoms return after you stop taking this medicine.
The symptoms of reflux may return after stopping this medicine suddenly, especially
if you have taken it for a while.
Things you must not do
Do not use this medicine to treat any other complaints unless your doctor tells you
to.
Do not give this medicine to anyone else, even if they have the same symptoms as you.
Things that may help your condition
Some self help measures suggested below may help your condition. Your doctor or pharmacist
can give you more information about these measures.
Alcohol –
your doctor may advise you to limit your alcohol intake.
Aspirin and many other medicines used to treat arthritis, period pain, headaches –
these medicines may irritate the stomach and may make your condition worse. Your doctor
or pharmacist may suggest other medicines you can take.
Caffeine –
your doctor may advise you to limit the number of drinks which contain caffeine, such
as coffee, tea, cocoa and cola drinks, because they contain ingredients that may irritate
your stomach.
Eating habits –
eat smaller, more frequent meals. Eat slowly and chew your food carefully. Try not
to rush at meal times.
Smoking –
your doctor may advise you to stop smoking or at least cut down.
Weight –
your doctor may suggest losing some weight to help your condition.
Side effects
Tell your doctor or pharmacist as soon as possible if you do not feel well while you
are taking Somac.
It helps most people peptic ulcers or reflux disease, but it may have unwanted side
effects in a few people.
All medicines have some unwanted side effects. Sometimes they are serious, but most
of the time they are not. You may need medical attention if you get some of the side-effects.
Ask your doctor or pharmacist any questions you may have.
Tell your doctor or pharmacist if you notice any of the following and they worry you:
headache
dizziness
diarrhoea
nausea or vomiting
stomach pain
excessive gas in the stomach or bowel
indigestion
constipation
dry mouth
metallic taste
weakness or tiredness
increased sweating or body temperature
blurred vision
skin problems such as itchiness and rash
trouble sleeping
These are the more common side effects of Somac.
Tell your doctor immediately if you notice any of the following:
unusual tiredness or weakness
nausea, vomiting, loss of appetite, feeling generally unwell, fever, itching, yellowing
of the skin and eyes, and dark coloured urine
blood in the urine
increased or decreased need to urinate
severe skin problems such as itchiness, redness, rash with swelling, blistering or
peeling of the skin or rash when exposed to the sun, possibly with pain in the joints
and general fever
swelling of the face, lips, mouth, tongue or throat which may cause difficulty in
swallowing or breathing
frequent infections such as fever, severe chills, sore throat or mouth ulcers
chest pain
shortness of breath
high blood pressure
water retention, swelling
bleeding or bruising more easily than normal
depression, confusion or anxiety
bone fracture of the hip, wrist or spine (mainly a risk in people who take high doses
of PPIs or use them long term (a year or longer))
symptoms such as seizures, abnormal or fast heartbeat, jerking/shaking movements or
muscle cramps. These can be a sign of low magnesium, calcium or potassium levels in
your blood
severe and/or persistent diarrhoea, because this medicine has been associated with
a small increase in infectious diarrhoea
The above list includes serious side effects that may require medical attention. Serious
side effects are rare.
Tell your doctor if you notice anything else that is making you feel unwell. Other
side effects not listed above may occur in some people.
Ask your doctor or pharmacist if you do not understand anything in this list.
Do not be alarmed by this list of possible side effects. You may not experience any
of them.
After taking Somac
Storage
Keep your tablets in their blister or bottle, and granules in their sachet pack until
it is time to take them.
If you take the tablets or granules out of the blister, bottle or sachet pack they
may not keep well.
Keep the medicine in a cool dry place where the temperature stays below 25°C (granules)
or 30°C (tablets).
Do not store it or any other medicines in the bathroom, near a sink or windowsill.
Do not leave it in the car.
Heat and dampness can destroy some medicines.
Keep it where children cannot reach it.
A locked cupboard at least one-and-a-half metres (5 feet) above the ground, is a good
place to store medicines.
Disposal
If your doctor tells you to stop taking this medicine or if the medicines have passed
their expiry date, ask your pharmacist what to do with any tablets or granules that
are left over.

Somac 20 mg tablets — yellow and oval shaped, marked with ‘P20’ on one side.

Фармакологические свойства
Фармакокинетика
Эзомепразол
неустойчив в кислой среде, поэтому для перорального применения
используют таблетки, содержащие гранулы препарата, оболочка которых
устойчива к действию желудочного сока. Препарат быстро абсорбируется:
максимальная концентрация в плазме достигается через 1-2 часа после
приема. Абсолютная биодоступность эзомепразола после однократного
приема дозы 40 мг составляет 64% и возрастает до 89% на фоне
ежедневного приема один раз в сутки. Для дозы 20 мг эзомепразола эти
показатели составляют 50% и 68%, соответственно. Объем распределения
при равновесной концентрации составляет приблизительно 0,22 л/кг
массы тела. Эзомепразол связывается с белками плазмы на 97%.
Прием
пищи замедляет и снижает всасывание эзомепразола в желудке, однако
это не оказывает существенного влияния на эффективность ингибирования
секреции соляной кислоты.
Эзомепразол
подвергается метаболизму с участием системы цитохрома Р450 (CYP).
Основная часть метаболизируется при участии специфической полиморфной
изоформы СYР2С19, при этом образуются гидрокси- и деметилированные
метаболиты эзомепразола. Метаболизм оставшейся части осуществляется
другой специфической изоформой CYP3A4; при этом образуется
сульфопроизводное эзомепразола, являющейся основным метаболитом,
определяемым в плазме.
При
ежедневном приеме один раз в сутки эзомепразол полностью выводится из
плазмы крови в перерыве между приемами и не кумулирует.
Основные
метаболиты эзомепразола не влияют на секрецию желудочной кислоты. При
пероральном применении до 80% дозы выводится в виде метаболитов с
мочой, остальное количество выводится с фекалиями. В моче
обнаруживается менее 1% неизмененного эзомепразола.
Особенности
фармакокинетики в некоторых группах пациентов
Приблизительно
у 2,9±1,5% населения снижена активность изофермента CYP2C19; у
таких пациентов метаболизм эзомепразола, в основном, осуществляется в
результате действия CYP3A4. При систематическом приеме 40 мг
эзомепразола однократно в сутки среднее значение АUС на 100%
превышает значение этого параметра у пациентов с повышенной
активностью изофермента CYP2C19. Средние значения максимальных
концентраций в плазме у пациентов со сниженной активностью
изофермента повышены приблизительно на 60%. Указанные особенности не
влияют на дозу и способ применения эзомепразола.
Особенности
фармакокинетики у пациентов пожилого возраста
У
пациентов пожилого возраста (71-80 лет) метаболизм эзомепразола не
претерпевает значительных изменений.
После
однократного приема 40 мг эзомепразола среднее значение АUС у женщин
на 30% выше чем у мужчин. При ежедневном приеме препарата один раз в
сутки различий в фармакокинетике у мужчин и женщин не отмечается.
Указанные особенности не влияют на дозу и способ применения
эзомепразола.
Особенности
фармакокинетики у пациентов с печеночной
недостаточностью
У
пациентов с легкой и умеренной печеночной недостаточностью метаболизм
эзомепразола может нарушаться. У пациентов с тяжелой печеночной
недостаточностью скорость метаболизма снижена, что приводит к
увеличению значения АUС для эзомепразола в 2 раза.
Особенности
фармакокинетики у пациентов с почечной
недостаточностью
Изучение
фармакокинетики у пациентов с почечной недостаточностью не
проводилось.
Особенности
фармакокинетики у детей
У
детей в возрасте 12-18 лет после повторного приема 20 мг и 40 мг
эзомепразола значение АUС и время достижения максимальной
концентрации (tmax) в плазме крови было сходно со значениями АUС и
tmax у взрослых.
Фармакодинамика
Сомпраз
снижает секрецию кислоты в желудке путем специфического ингибирования
протонного насоса в париетальных клетках, является S-изомером
омепразола. S—
и R-изомеры
омепразола обладают сходной фармакодинамической активностью.
Механизм
действия
Сомпраз
является слабым основанием, накапливается в секреторных канальцев
париетальных клеток в кислой среде, где активируется и ингибирует
протонный насос – фермент Н+,К+-АТФазу.
Эзомепразол ингибирует как базальную, так и стимулированную секрецию
кислоты. Действие эзомепразола развивается в течение 1 часа после
перорального приема 20 мг или 40 мг. При ежедневном приеме препарата
в течение 5 дней в дозе 20 мг один раз в сутки средняя максимальная
концентрация кислоты после стимуляции пентагастрином снижается на 90%
(при измерении концентрации кислоты через 6-7 часов после приема
препарата на 5-й день терапии).
Терапевтический
эффект, достигаемый в результате ингибирования секреции кислоты
При
приеме Сомпраза в дозе 40 мг заживление рефлюкс-эзофагита наступает
приблизительно у 78% пациентов через 4 недели терапии и у 93% — через
8 недель терапии.
Пациентам
с неосложненной язвенной болезнью после недельного эрадикационного
курса не требуется последующей монотерапии антисекреторными
препаратами для заживления язвы и устранения симптомов.
Показания к применению

пептической язвы (после внутривенного применения препаратов,

Способ применения и дозы
Внутрь.
Таблетку следует проглатывать целиком, запивая жидкостью. Таблетки
нельзя разжевывать или разламывать.
Лечение
эрозивного рефлюкс-эзофагита: по 40 мг 1 раз в сутки в течение 4 нед.
Рекомендуется
дополнительный 4-недельный курс лечения в тех случаях, когда после
первого курса излечение эзофагита не наступает или симптомы остаются.
Профилактика
рецидивов у пациентов с излеченным эзофагитом: по 20 мг 1 раз в
сутки.
Симптоматическое
лечение ГЭРБ:
20 мг 1 раз в сутки — пациентам без эзофагита. Если после
4 нед лечения
симптомы не исчезают, следует провести дополнительное обследование
пациента.
В
комбинации с соответствующей антибактериальной терапией для
эрадикации Helicobacter
pylori.
Внутрь.
Таблетку следует глотать целиком, запивая водой. Таблетки нельзя
разжевывать или дробить. Для пациентов с затруднением глотания можно
растворять таблетки в половине стакана негазированной воды (не
следует использовать другие жидкости, так как защитная оболочка
микрогранул может раствориться), размешивая до растворения таблетки,
после чего взвесь микрогранул следует выпить сразу или в течение 30
минут, после чего снова наполнить стакан водой наполовину, размешать
остатки и выпить. Пациенты, которые не могут глотать, таблетки
следует растворять в воде и вводить через назогастральный зонд.
Взрослые
и дети с 12 лет
Гастро-эзофагеальная
рефлюксная болезнь
По
40 мг один раз в сутки в течение 4-х недель. Рекомендуется
дополнительный 4-х недельный курс лечения в случаях, когда после
первого курса излечение эзофагита не наступает или сохраняются
симптомы.
Длительное
поддерживающее лечение после заживления эрозивного рефлюкс-эзофагита
для предотвращения рецидива (профилактика
рецидивов)
по
20 мг один раз в сутки.
Симптоматическое
лечение гастроэзофагеальной рефлюксной болезни
20
мг один раз в сутки – пациентам без эзофагита. Если после 4-х
недель лечения симптомы не исчезают, следует провести дополнительное
обследование пациента.
Длительная
кислотоподавляющая терапия у пациентов, перенесших кровотечение из
пептической язвы (после внутривенного применения препаратов,
понижающих секрецию желез желудка, для профилактики рецидива)
Сомпраз
40 мг 1 раз в сутки в течение 4 недель после окончания внутривенной
терапии препаратами, понижающими секрецию желез желудка.
Состояния,
связанные с патологической гиперсекрецией, в том числе, синдром
Золлингера-Эллисона и идиопатическая гиперсекреция
Рекомендуемая
начальная доза — Сомпраз 40 мг два раза в сутки.
В
дальнейшем доза подбирается индивидуально, длительность лечения
определяется клинической картиной заболевания.
Пациентам
с почечной недостаточностью и для пациентов пожилого возраста
коррекции дозы не требуется.
При
тяжелой печеночной недостаточности доза не должна превышать 20
мг/сут.
Введение
препарата через назогастральный зонд
1.
Поместите таблетку в шприц и заполните шприц 25 мл воды и
приблизительно 5 мл воздуха. Для некоторых зондов может потребоваться
разведение препарата в 50 мл питьевой воды для того, чтобы
предотвратить засорение зонда гранулами таблетки.
2.
Взболтайте содержимое шприца в течение примерно двух минут для
растворения таблетки.
3.
Держите шприц наконечником вверх и убедитесь, что наконечник не
засорился.
4.
Введите наконечник шприца в зонд, продолжая удерживать его
направленным вверх.
5.
Встряхните шприц и переверните его наконечником вниз. Немедленно
введите 5-10 мл растворенного препарата в зонд. После введения
верните шприц в прежнее положение и взболтайте (шприц должен
удерживаться наконечником вверх для избежания засорения наконечника).
6.
Переверните шприц наконечником вниз и введите еще 5-10 мл препарата в
зонд. Повторите данную операцию, пока шприц не будет пуст.
7.
В случае остатка части препарата в виде осадка в шприце заполните
шприц 25 мл воды и 5 мл воздуха и повторите операции, описанные в
пункте 5.

Лекарственные взаимодействия
Эзомепразол
ингибирует CYP2C19 и CYP3A4
—
основной фермент, участвующий в его метаболизме. Соответственно,
совместное применение эзомепразола с другими препаратами, в
метаболизме которых принимает участие CYP2C19, такими как диазепам,
циталопрам, имипрамин, кломипрамин, фенитоин и др., может привести к
повышению концентраций этих препаратов в плазме, что, в свою очередь,
может потребовать снижения дозы. Об этом взаимодействии особенно
важно помнить при назначении Сомпраза в режиме «по
необходимости». При совместном приеме 30 мг эзомепразола и
диазепама, который является субстратом цитохрома CYP2C19, отмечается
снижение клиренса диазепама на 45%.
Назначение
эзомепразола в дозе 40 мг приводило к повышению остаточной
концентрации фенитоина у пациентов с эпилепсией на 13%. В связи с
этим рекомендуется контролировать концентрации фенитоина в плазме в
начале лечения эзомепразолом и при его отмене.
Следует
избегать совместного применения эзомепразола и клопидогреля.
Подавление
кислотности желудочного сока на фоне лечения эзомепразолом и другими
ингибиторами протонной помпы (ИПП) может привести к изменению
абсорбции препаратов, всасывание которых зависит от кислотности
среды. Эзомепразол, как и другие препараты, снижающие секрецию
кислоты в желудке, может приводить к снижению абсорбции кетоконазола,
итраконазола и эрлотиниба. В то же время при совместном приеме
эзомепразола с дигоксином, концентрация последнего может
увеличиваться. При совместном приеме ИПП и метотрексата, отмечается
увеличение концентрации у некоторых пациентов. При применении
метотрексата в высоких дозах необходимо приостановить прием
эзомепразола.
В
случаях, когда атазанавир и нелфинавир принимались одновременно с
омепразолом, было отмечено понижение уровня этих препаратов в
сыворотке, поэтому их сочетанное применение следует избегать.
Совместное назначение омепразола в дозе 40 мг в один раз сутки и
атазанавира 300 мг/ритонавира 100 мг приводило к существенному
снижению значений AUC, а также максимальной и минимальной
концентраций атазанавира. Увеличение дозы атазанавира до 400 мг не
компенсировало влияния омепразола на концентрацию атазанавира.
Поэтому не следует назначать эзомепразол совместно с атазанавиром, а
одновременное назначение эзомепразола и нелфинавира исключается.
Совместный
прием варфарина с 40 мг эзомепразола не приводит к изменению времени
коагуляции у пациентов, длительно принимающих варфарин. Однако
сообщалось о нескольких случаях клинически значимого повышения
индекса МНО (международное нормализованное отношение) при совместном
применении варфарина и эзомепразола. Поэтому в начале и по окончании
совместного применения этих препаратов рекомендуется контролировать
МНО.
Совместный
прием цизаприда с 40 мг эзомепразола приводит к повышению значений
фармакокинетических параметров цизаприда у здоровых добровольцев: AUC
— на 32% и периода полувыведения на 31%, однако максимальная
концентрация цизаприда в плазме при этом значительно не изменяется.
Незначительное удлинение интервала QT, которое наблюдалось при
монотерапии цизапридом, при добавлении Сомпраза не увеличивалось.
Сомпраз
не вызывает клинически значимых изменений фармакокинетики
амоксициллина и хинидина.
Совместное
назначение с эзомепразолом приводило к увеличению концентрации
такролимуса в сыворотке.
Исследования
по оценке краткосрочного совместного применения эзомепразола и
напроксена или рофекоксиба не выявили клинически значимого
фармакокинетического взаимодействия.
Особые указания
При
появлении любых тревожных симптомов (например, таких как значительная
спонтанная потеря массы тела, повторная рвота, дисфагия, рвота с
примесью крови или мелена), а также при наличии язвы желудка (или при
подозрении на язву желудка) следует исключить наличие
злокачественного новообразования, поскольку лечение Сомпразом может
привести к сглаживанию симптоматики и отсрочить постановку диагноза.
Пациенты,
принимающие препарат в течение длительного периода (особенно более
года), должны находиться под регулярным наблюдением врача.
Пациенты,
принимающие Сомпраз «по необходимости», должны быть
проинструктированы о необходимости связаться со своим врачом при
изменении характера симптомов. Принимая во внимание колебания
концентрации эзомепразола в плазме при назначении терапии «по
необходимости», следует учитывать взаимодействие препарата с
другими лекарственными средствами.
При
назначении Сомпраза для эрадикации Helicobacter
pylori должна
учитываться возможность лекарственных взаимодействий для всех
компонентов тройной терапии. Кларитромицин является мощным
ингибитором CYP3А4, поэтому при назначении эрадикационной терапии
пациентам, получающим другие препараты, метаболизирующиеся с участием
CYP3А4 (например, цисаприда) необходимо учитывать возможные
противопоказания и взаимодействия кларитромицина с этими
лекарственными средствами.
Таблетки
Сомпраза содержат сахарозу, поэтому они противопоказаны пациентам с
наследственной непереносимостью фруктозы, глюкозо-галактозной
мальабсорбцией или сахаразо-изомальтазной недостаточностью.
Лечение
ингибиторами протоновой помпы может привести к незначительному
увеличению риска возникновения желудочно-кишечных инфекций,
вызываемых Salmonella и Campylobacter.
У
пациентов, принимающих антисекреторные препараты в течение
длительного промежутка времени, чаще отмечается образование
железистых кист в желудке. Эти явления обусловлены физиологическими
изменениями в результате выраженного ингибирования секреции кислоты.
Кисты доброкачественные и носят обратимый характер.
Эзомепразол
может уменьшить всасывание витамина В12.
Применение более трех месяцев может привести к серьезным проявлениям
гипомагнезиемии (усталость, судороги, головокружение, желудочковой
аритмии, тетании, делирии). Применение длительно, в больших дозах
увеличивает риск развития переломов бедра, запястья, позвоночника,
преимущественно у лиц пожилого возраста или в присутствии других
факторов риска. Если на фоне приема ингибиторов протонной помпы
возникают повреждения кожи, особенно на открытых участках,
сопровождающиеся артралгией, пациент должен обратиться за медицинской
помощью, для исключения дискоидной красной волчанки.
Во
время лечения ингибиторами протонового насоса концентрация гастрина в
плазме повышается в результате сниженной внутрижелудочной секреции
HCl.
Почечная
недостаточность: коррекция
дозы препарата не требуется.
Опыт
применения Сомпраза у пациентов с тяжелой почечной недостаточностью
ограничен; в связи с этим, при назначении препарата таким пациентам
следует соблюдать осторожность.
Печеночная
недостаточность: при
легкой и умеренной печеночной недостаточности коррекция дозы
препарата не требуется.
Для
пациентов с тяжелой печеночной недостаточностью не следует превышать
максимальную суточную дозу — 20 мг.
Пациенты
пожилого возраста: коррекция дозы препарата не требуется.
Беременность
и лактация
Назначать
препарат беременным следует только в том случае, когда ожидаемая
польза для матери превышает возможный риск для плода.
Неизвестно
выделяется ли эзомепразол с грудным молоком, поэтому не следует
назначать Сомпраз во время кормления грудью.
Особенности
влияния лекарственного средства на способность управлять транспортным
средством или потенциально опасными механизмами
Учитывая
побочные действия лекарственного средства, следует соблюдать
осторожность при управлении транспортным средством или потенциально
опасными механизмами.
Передозировка
Симптомы: усиление
симптомов побочных эффектов
Лечение: симптоматическое.
Специфические антидоты неизвестны. Эзомепразол связывается с белками
плазмы, поэтому диализ малоэффективен.
Форма выпуска и упаковка
По
7 таблеток помещают в контурную ячейковую упаковку из пленки
полиамид/ алюминий/поливинилхлорид.
По
2 контурных упаковки вместе с инструкцией по медицинскому применению
на русском и государственном языках помещают в картонную пачку.

Хранить
в сухом месте при температуре не выше 25С.
