При применении пантопразола, лиофилизат для приготовления раствора для в/в введения, в соответствии с показаниями и в рекомендованных дозах побочные эффекты возникают крайне редко. Наиболее частой нежелательной побочной реакцией является тромбофлебит в месте введения препарата. Диарея и головная боль наблюдаются примерно у 1% пациентов.

PRECAUTIONS
Presence Of Gastric Malignancy
In adults, symptomatic response to therapy with PROTONIX does not preclude the presence of gastric malignancy. Consider additional follow-up and diagnostic testing in adult patients who have a suboptimal response or an early symptomatic relapse after completing treatment with a PPI. In older patients, also consider an endoscopy.
Acute Interstitial Nephritis
Acute interstitial nephritis has been observed in patients taking PPIs including PROTONIX. Acute interstitial nephritis may occur at any point during PPI therapy and is generally attributed to an idiopathic hypersensitivity reaction. Discontinue PROTONIX if acute interstitial nephritis develops.
Clostridium Difficile-Associated Diarrhea
Published observational studies suggest that PPI therapy like PROTONIX may be associated with an increased risk of Clostridium difficile associated diarrhea, especially in hospitalized patients. This diagnosis should be considered for diarrhea that does not improve.
Patients should use the lowest dose and shortest duration of PPI therapy appropriate to the condition being treated.
Bone Fracture
Several published observational studies suggest that PPI therapy may be associated with an increased risk for osteoporosis-related fractures of the hip, wrist, or spine. The risk of fracture was increased in patients who received high-dose, defined as multiple daily doses, and longterm PPI therapy (a year or longer). Patients should use the lowest dose and shortest duration of PPI therapy appropriate to the condition being treated. Patients at risk for osteoporosis-related fractures should be managed according to established treatment guidelines.
Cutaneous And Systemic Lupus Erythematosus
Cutaneous lupus erythematosus (CLE) and systemic lupus erythematosus (SLE) have been reported in patients taking PPIs, including pantoprazole sodium. These events have occurred as both new onset and an exacerbation of existing autoimmune disease. The majority of PPIinduced lupus erythematous cases were CLE.
The most common form of CLE reported in patients treated with PPIs was subacute CLE (SCLE) and occurred within weeks to years after continuous drug therapy in patients ranging from infants to the elderly. Generally, histological findings were observed without organ involvement.
Systemic lupus erythematosus (SLE) is less commonly reported than CLE in patients receiving PPIs. PPI associated SLE is usually milder than non-drug induced SLE. Onset of SLE typically occurred within days to years after initiating treatment primarily in patients ranging from young adults to the elderly. The majority of patients presented with rash; however, arthralgia and cytopenia were also reported.
Avoid administration of PPIs for longer than medically indicated. If signs or symptoms consistent with CLE or SLE are noted in patients receiving PROTONIX, discontinue the drug and refer the patient to the appropriate specialist for evaluation. Most patients improve with discontinuation of the PPI alone in 4 to 12 weeks. Serological testing (e.g. ANA) may be positive and elevated serological test results may take longer to resolve than clinical manifestations.
Cyanocobalamin (Vitamin B-12) Deficiency
Generally, daily treatment with any acid-suppressing medications over a long period of time (e.g., longer than 3 years) may lead to malabsorption of cyanocobalamin (Vitamin B-12) caused by hypo- or achlorhydria. Rare reports of cyanocobalamin deficiency occurring with acid-suppressing therapy have been reported in the literature. This diagnosis should be considered if clinical symptoms consistent with cyanocobalamin deficiency are observed.
Hypomagnesemia
Hypomagnesemia, symptomatic and asymptomatic, has been reported rarely in patients treated with PPIs for at least three months, in most cases after a year of therapy. Serious adverse events include tetany, arrhythmias, and seizures. In most patients, treatment of hypomagnesemia required magnesium replacement and discontinuation of the PPI.
For patients expected to be on prolonged treatment or who take PPIs with medications such as digoxin or drugs that may cause hypomagnesemia (e.g., diuretics), health care professionals may consider monitoring magnesium levels prior to initiation of PPI treatment and periodically.
Tumorigenicity
Due to the chronic nature of GERD, there may be a potential for prolonged administration of PROTONIX. In long-term rodent studies, pantoprazole was carcinogenic and caused rare types of gastrointestinal tumors. The relevance of these findings to tumor development in humans is unknown.
Interference With Urine Screen For THC
There have been reports of false positive urine screening tests for tetrahydrocannabinol (THC) in patients receiving PPIs, including PROTONIX.
Concomitant Use Of PROTONIX With Methotrexate
Literature suggests that concomitant use of PPIs with methotrexate (primarily at high dose; see methotrexate prescribing information) may elevate and prolong serum levels of methotrexate and/or its metabolite, possibly leading to methotrexate toxicities. In high-dose methotrexate administration, a temporary withdrawal of the PPI may be considered in some patients .

Drug Interactions
Instruct patients to inform their healthcare provider of any other medications they are currently taking, including rilpivirine-containing products , high dose methotrexate and over-the-counter medications.
Pregnancy
Inform female patients of reproductive potential that PROTONIX may cause fetal harm and to inform their prescriber of a known or suspected pregnancy.
Administration
- Caution patients that PROTONIX For Delayed-Release Oral Suspension and PROTONIX Delayed-Release Tablets should not be split, crushed, or chewed.
- PROTONIX oral suspension packet is a fixed dose and cannot be divided to make a smaller dose.
- Tell patients that PROTONIX Delayed-Release Tablets should be swallowed whole, with or without food in the stomach.
- Let patients know that concomitant administration of antacids does not affect the absorption of PROTONIX Delayed-Release Tablets.
- Advise patients to take PROTONIX For Delayed-Release Oral Suspension approximately 30 minutes before a meal.
- Advise patients that PROTONIX For Delayed-Release Oral Suspension should only be administered in apple juice or applesauce, not in water, other liquids, or foods.
Nonclinical Toxicology
Carcinogenesis, Mutagenesis, Impairment Of Fertility
In a 24-month carcinogenicity study, Sprague-Dawley rats were treated orally with pantoprazole doses of 0.5 to 200 mg/kg/day, about 0.1 to 40 times the exposure on a body surface area basis of a 50 kg person dosed with 40 mg/day. In the gastric fundus, treatment at 0.5 to 200 mg/kg/day produced enterochromaffin-like (ECL) cell hyperplasia and benign and malignant neuroendocrine cell tumors in a dose-related manner. In the forestomach, treatment with 50 and 200 mg/kg/day (about 10 and 40 times the recommended human dose on a body surface area basis) produced benign squamous cell papillomas and malignant squamous cell carcinomas. Rare gastrointestinal tumors associated with pantoprazole treatment included an adenocarcinoma of the duodenum with 50 mg/kg/day and benign polyps and adenocarcinomas of the gastric fundus with 200 mg/kg/day. In the liver, treatment at 0.5 to 200 mg/kg/day produced dose-related increases in the incidences of hepatocellular adenomas and carcinomas. In the thyroid gland, treatment with 200 mg/kg/day produced increased incidences of follicular cell adenomas and carcinomas for both male and female rats.
In a 24-month carcinogenicity study, Fischer 344 rats were treated orally with doses of 5 to 50 mg/kg/day of pantoprazole, approximately 1 to 10 times the recommended human dose based on body surface area. In the gastric fundus, treatment with 5 to 50 mg/kg/day produced enterochromaffin-like (ECL) cell hyperplasia and benign and malignant neuroendocrine cell tumors. Dose selection for this study may not have been adequate to comprehensively evaluate the carcinogenic potential of pantoprazole.
In a 24-month carcinogenicity study, B6C3F1 mice were treated orally with doses of 5 to 150 mg/kg/day of pantoprazole, 0.5 to 15 times the recommended human dose based on body surface area. In the liver, treatment with 150 mg/kg/day produced increased incidences of hepatocellular adenomas and carcinomas in female mice. Treatment with 5 to 150 mg/kg/day also produced gastric-fundic ECL cell hyperplasia.
A 26-week p53 +/- transgenic mouse carcinogenicity study was not positive.
Pantoprazole was positive in the in vitro human lymphocyte chromosomal aberration assays, in one of two mouse micronucleus tests for clastogenic effects, and in the in vitro Chinese hamster ovarian cell/HGPRT forward mutation assay for mutagenic effects. Equivocal results were observed in the in vivo rat liver DNA covalent binding assay. Pantoprazole was negative in the in vitro Ames mutation assay, the in vitro unscheduled DNA synthesis (UDS) assay with rat hepatocytes, the in vitro AS52/GPT mammalian cell-forward gene mutation assay, the in vitro thymidine kinase mutation test with mouse lymphoma L5178Y cells, and the in vivo rat bone marrow cell chromosomal aberration assay.
There were no effects on fertility or reproductive performance when pantoprazole was given at oral doses up to 500 mg/kg/day in male rats (98 times the recommended human dose based on body surface area) and 450 mg/kg/day in female rats (88 times the recommended human dose based on body surface area).

Pregnancy
Teratogenic Effects
Pregnancy Category C
Reproduction studies have been performed in rats at oral pantoprazole doses up to 450 mg/kg/day (about 88 times the recommended human dose based on body surface area) and in rabbits at oral doses up to 40 mg/kg/day (about 16 times the recommended human dose based on body surface area) with administration of pantoprazole sodium during organogenesis in pregnant animals. The studies have revealed no evidence of impaired fertility or harm to the fetus due to pantoprazole.
A pre- and postnatal development toxicity study in rats with additional endpoints to evaluate the effect on bone development was performed with pantoprazole sodium. Oral pantoprazole doses of 5, 15, and 30 mg/kg/day (approximately 1, 3, and 6 times the human dose of 40 mg/day on a body surface area basis) were administered to pregnant females from gestation day (GD) 6 through lactation day (LD) 21. On postnatal day (PND 4) through PND 21, the pups were administered oral doses at 5, 15, and 30 mg/kg/day (approximately 1, 2.3, and 3.2 times the exposure (AUC) in humans at a dose of 40 mg). There were no drug-related findings in maternal animals. During the preweaning dosing phase (PND 4 to 21) of the pups, there were increased mortality and/or moribundity and decreased body weight and body weight gain at 5 mg/kg/day (approximately equal exposures (AUC) in humans receiving the 40 mg dose) and higher doses. On PND 21, decreased mean femur length and weight and changes in femur bone mass and geometry were observed in the offspring at 5 mg/kg/day (approximately equal exposures (AUC) in humans at the 40 mg dose) and higher doses. The femur findings included lower total area, bone mineral content and density, periosteal and endosteal circumference, and cross-sectional moment of inertia. There were no microscopic changes in the distal femur, proximal tibia, or stifle joints. Changes in bone parameters were partially reversible following a recovery period, with findings on PND 70 limited to lower femur metaphysis cortical/subcortical bone mineral density in female pups at 5 mg/kg/day (approximately equal exposures (AUC) in humans at the 40 mg dose) and higher doses.
There are no adequate and well-controlled studies in pregnant women. Advise pregnant women of the potential risk of fetal harm. Because animal reproduction studies are not always predictive of human response, this drug should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Nursing Mothers
Pantoprazole and its metabolites are excreted in the milk of rats. Pantoprazole excretion in human milk has been detected in a study of a single nursing mother after a single 40 mg oral dose of pantoprazole sodium. The clinical relevance of this finding is not known. Many drugs which are excreted in human milk have a potential for serious adverse reactions in nursing infants. Based on the potential for tumorigenicity shown for pantoprazole sodium in rodent carcinogenicity studies, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the benefit of the drug to the mother.
Pediatric Use
The safety and effectiveness of PROTONIX for short-term treatment (up to eight weeks) of erosive esophagitis (EE) associated with GERD have been established in pediatric patients 1 year through 16 years of age. Effectiveness for EE has not been demonstrated in patients less than 1 year of age. In addition, for patients less than 5 years of age, there is no appropriate dosage strength in an age-appropriate formulation available. Therefore, PROTONIX is indicated for the short-term treatment of EE associated with GERD for patients 5 years and older. The safety and effectiveness of PROTONIX for pediatric uses other than EE have not been established.
1 Year Through 16 Years Of Age
Use of PROTONIX in pediatric patients 1 year through 16 years of age for short-term treatment (up to eight weeks) of EE associated with GERD is supported by: a) extrapolation of results from adequate and well-controlled studies that supported the approval of PROTONIX for treatment of EE associated with GERD in adults, and b) safety, effectiveness, and pharmacokinetic studies performed in pediatric patients.
Safety of PROTONIX in the treatment of EE associated with GERD in pediatric patients 1 through 16 years of age was evaluated in three multicenter, randomized, double-blind, parallel-treatment studies, involving 249 pediatric patients, including 8 with EE (4 patients ages 1 year to 5 years and 4 patients 5 years to 11 years). The children ages 1 year to 5 years with endoscopically diagnosed EE (defined as an endoscopic Hetzel-Dent score ≥ 2) were treated once daily for 8 weeks with one of two dose levels of PROTONIX (approximating 0.6 mg/kg or 1.2 mg/kg). All 4 of these patients with EE were healed (Hetzel-Dent score of 0 or 1) at 8 weeks. Because EE is uncommon in the pediatric population, predominantly pediatric patients with endoscopically-proven or symptomatic GERD were also included in these studies.
Patients were treated with a range of doses of PROTONIX once daily for 8 weeks. For safety findings see ADVERSE REACTIONS. Because these pediatric trials had no placebo, active comparator, or evidence of a dose response, the trials were inconclusive regarding the clinical benefit of PROTONIX for symptomatic GERD in the pediatric population. The effectiveness of PROTONIX for treating symptomatic GERD in pediatric patients has not been established.
Although the data from the clinical trials support use of PROTONIX for the short-term treatment of EE associated with GERD in pediatric patients 1 year through 5 years, there is no commercially available dosage formulation appropriate for patients less than 5 years of age.
In a population pharmacokinetic analysis, clearance values in the children 1 to 5 years old with endoscopically proven GERD had a median value of 2.4 L/h. Following a 1.2 mg/kg equivalent dose (15 mg for. 12.5 kg and 20 mg for > 12.5 to < 25 kg), the plasma concentrations of pantoprazole were highly variable and the median time to peak plasma concentration was 3 to 6 hours. The estimated AUC for patients 1 to 5 years old was 37% higher than for adults receiving a single 40 mg tablet, with a geometric mean AUC value of 6.8 μg•hr/mL.
Neonates To Less Than One Year Of Age
PROTONIX was not found to be effective in a multicenter, randomized, double-blind, placebo-controlled, treatment-withdrawal study of 129 pediatric patients 1 through 11 months of age. Patients were enrolled if they had symptomatic GERD based on medical history and had not responded to non-pharmacologic interventions for GERD for two weeks. Patients received PROTONIX daily for four weeks in an open-label phase, then patients were randomized in equal proportion to receive PROTONIX treatment or placebo for the subsequent four weeks in a double-blind manner. Efficacy was assessed by observing the time from randomization to study discontinuation due to symptom worsening during the four-week treatment-withdrawal phase. There was no statistically significant difference between PROTONIX and placebo in the rate of discontinuation.
In this trial, the adverse reactions that were reported more commonly (difference of ≥ 4%) in the treated population compared to the placebo population were elevated CK, otitis media, rhinitis, and laryngitis.
In a population pharmacokinetic analysis, the systemic exposure was higher in patients less than 1 year of age with GERD compared to adults who received a single 40 mg dose (geometric mean AUC was 103% higher in preterm infants and neonates receiving single dose of 2.5 mg of PROTONIX, and 23% higher in infants 1 through 11 months of age receiving a single dose of approximately 1.2 mg/kg). In these patients, the apparent clearance (CL/F) increased with age (median clearance: 0.6 L/hr, range: 0.03 to 3.2 L/hr).
These doses resulted in pharmacodynamic effects on gastric but not esophageal pH. Following once daily dosing of 2.5 mg of PROTONIX in preterm infants and neonates, there was an increase in the mean gastric pH (from 4.3 at baseline to 5.2 at steady-state) and in the mean % time that gastric pH was > 4 (from 60% at baseline to 80% at steady-state). Following once daily dosing of approximately 1.2 mg/kg of PROTONIX in infants 1 through 11 months of age, there was an increase in the mean gastric pH (from 3.1 at baseline to 4.2 at steady-state) and in the mean % time that gastric pH was > 4 (from 32% at baseline to 60% at steady-state). However, no significant changes were observed in mean intraesophageal pH or % time that esophageal pH was < 4 in either age group.
Because PROTONIX was not shown to be effective in the randomized, placebo-controlled study in this age group, the use of PROTONIX for treatment of symptomatic GERD in infants less than 1 year of age is not indicated.
Animal Toxicity Data
In a pre- and post-natal development study in rats, the pups were administered oral doses of pantoprazole at 5, 15, and 30 mg/kg/day (approximately 1, 2.3, and 3.2 times the exposure (AUC) in children aged 6 to 11 years at a dose of 40 mg) on postnatal day (PND 4) through PND 21, in addition to lactational exposure through milk. On PND 21, decreased mean femur length and weight and changes in femur bone mass and geometry were observed in the offspring at 5 mg/kg/day (approximately equal exposures (AUC) in children aged 6 to 11 years at the 40 mg dose) and higher doses. Changes in bone parameters were partially reversible following a recovery period.
In neonatal/juvenile animals (rats and dogs) toxicities were similar to those observed in adult animals, including gastric alterations, decreases in red cell mass, increases in lipids, enzyme induction and hepatocellular hypertrophy. An increased incidence of eosinophilic chief cells in adult and neonatal/juvenile rats, and atrophy of chief cells in adult rats and in neonatal/juvenile dogs, was observed in the fundic mucosa of stomachs in repeated-dose studies. Full to partial recovery of these effects were noted in animals of both age groups following a recovery period.
Geriatric Use
In short-term US clinical trials, erosive esophagitis healing rates in the 107 elderly patients (≥ 65 years old) treated with PROTONIX were similar to those found in patients under the age of 65. The incidence rates of adverse reactions and laboratory abnormalities in patients aged 65 years and older were similar to those associated with patients younger than 65 years of age.

Одна таблетка содержит
активное вещество — пантопразола натрия сесквигидрат 22,62 мг, (эквивалентно пантопразолу 20,0 мг) и 45,310 мг, (эквивалентно пантопразолу 40,0 мг),
вспомогательные вещества: маннитол, натрия карбонат безводный, натрия крахмала гликолят, кросповидон, кремния диоксид коллоидный, кальция стеарат,
состав оболочки: гидроксипропилметилцеллюлоза , полиэтиленгликоль, вода очищенная, натрия гидроксид,
состав кишечнорастворимого покрытия: эудрагит L 30D55, полиэтилен гликоль, вода очищенная
состав вторичной оболочки опадри АМВ 80W 52172, вода очищенная.
Описание
Овальные двояковыпуклые таблетки, покрытые пленочной оболочкой желтого цвета.
Фармакотерапевтическая группа
Препараты для лечения заболеваний, связанных с нарушением кислотности. Противоязвенные препараты и препараты для лечения гастроэзофагального рефлюкса. Ингибиторы протонового насоса. Пантопразол.

Фармакологические свойства
Фармакокинетика
Пантопразол быстро абсорбируется из желудочно-кишечного тракта. Абсолютная биодоступность составляет 77%. Максимальная концентрация в плазме крови после приема дозы 20 мг — 2 – 3мкг/мл достигается через 2,5 часа и остается постоянной после многократного применения. Объем распределения – приблизительно 0,15 л/ч/кг, клиренс – около 0,1 л/ч/кг.
Связывание с белками плазмы крови – приблизительно 98%. Пантопразол практически полностью метаболизируется в печени. Основной метаболит, определяемый в сыворотке крови и моче — десметилпантопразол, который соединяется сульфатом. Период полувыведения составляет примерно 1 час. Приблизительно 80% препарата элиминируется почками, остальные 20% с фекалиями.
Фармакодинамика
Пантопразол является замещенным бензимидазолом, замедляющим секрецию соляной кислоты в желудке путем специфического воздействия на протонную помпу париетальных клеток.
В кислой среде париетальных клеток пантопразол превращается в свою активную форму – циклический сульфенамид, где он подавляет фермент H+, K+-АТФазу.
Блокируя протонный насос, пантопразол вмешивается в процесс кислотообразования на уровне конечного этапа синтеза соляной кислоты в желудке. Ингибирующее действие дозозависимо и влияет на уровень базальной и стимулированной секреции соляной кислоты (независимо от вида раздражителя). При язвенной болезни желудка и двенадцатиперстной кишки, ассоциированной с Неlicobacter pylori, такое снижение желудочной секреции повышает чувствительность микроорганизма к антибиотикам.
После перорального приема 20 мг препарата Пантонекс –DR 20 антисекреторный эффект наступает через 1 час и достигает максимума через 2 – 4 часа. Не влияет на моторику желудочно-кишечного тракта. Секреторная активность возвращается к исходному уровню через 3 – 4 дня после окончания приема.
Пантопразол имеет большую химическую стабильность при нейтральном рН, и меньший потенциал взаимодействия с другими лекарственными препаратами.
Показания к применению
— кратковременное лечение рефлюксных симптомов, таких как изжога, отрыжка кислым
Способ применения и дозы
Кишечнорастворимые таблетки Пантонекс- DR 20, Пантонекс- DR 40 не следует жевать или раскусывать; они должны проглатываться целиком и запиваться достаточным количеством жидкости. Прием препарата необходимо осуществлять перед едой.
При назначении двукратного приема препарата, вторая таблетка должна быть употреблена перед вечерним приемом пищи.
Симптоматическое улучшение у больных может начинаться приблизительно через один день лечения Пантонекс- DR 20, но может потребоваться 7 дней для достижения полного исчезновения симптомов.
Кратковременное лечение рефлюксных симптомов (изжога, отрыжка кислым)
Взрослым назначают по 20 мг в сутки.
Для купирования симптомов может быть необходимо, принимать препарат в течение 2 – 3 дней. После того, как симптомы исчезли, проводимое лечение может быть прекращено. Если улучшения не наблюдается в течение 2 недель непрерывного лечения, пациенту необходимо обратиться к врачу.
Побочные действия
Ниже перечислены наблюдавшиеся побочные реакции с частотой их встречаемости: очень часто ( 1/10); часто (от 1/00 до 1/10); нечасто (от 1/1000 до 1/100); редко (от 1/10000 до 1/1000); очень редко ( 1/10000); неизвестно (невозможно оценить по имеющимся данным).

Противопоказания
— гиперчувствительность к компонентам препарата, другим замещенным бензимидазолам
— совместное применение пантопразола с атазанавиром
— беременность и период лактации
— детский возраст до 18 лет
Лекарственные взаимодействия
Следует обратить внимание на одновременный прием с препаратами, всасывание которых является рН-зависимым, например кетоконазолом, итраконазолом, позаконазолом, эрлотинибом, в том числе с препаратами, назначавшимися незадолго до курса лечения Пантонекс- DR 20, ввиду изменения абсорбции данных препаратов.
Одновременный прием Пантонекс- DR 20 и атазанавира значительно снижает эффективность последнего.
При совместном применении фенпрокумона или варфарина, было отмечено несколько единичных случаев изменения Международного нормализационного отношения (МНО или INR). Поэтому для больных, проходящих лечение кумариновыми антикоагулянтами (например, фенпрокумоном или варфарином), рекомендуется следить за протромбиновым временем/МНО после начала, окончания или во время нерегулярного применения Пантонекс- DR 20.
Пантонекс- DR 20 обладает низким потенциалом взаимодействия с системой цитохрома Р450. Поэтому риск побочных реакций, вызванных взаимодействием Пантонекс- DR 20 с другими препаратами, метаболизирующимся системой цитохром Р450, может считаться минимальным. Специальные исследования не выявили клинически значимого взаимодействия препарата Пантонекс- ДР 40 с кофеином, карбамазепином, диазепамом, этанолом, диклофенаком, глибенкламидом, напроксеном, метопрололом, нифедипином, фенитоином, пироксикамом, теофиллином, оральными контрацептивами, содержащими левоноргестрел и этинилэстрадиол.
При одновременном приеме с антибиотиками, такими как кларитромицин, амоксициллин, с метронидазолом, взаимодействия не выявлено.
Взаимодействия с антацидами при одновременном приеме не выявлено.
Особые указания
У пациентов с тяжелыми нарушениями функции печени при приеме препарата Пантонекс- DR 20, особенно в течении длительного периода, рекомендуется проводить регулярный контроль уровня ферментов печени. При увеличении уровня печеночных ферментов следует прекратить применение препарата.
Пациентам, нуждающимся в продолжительном лечении неспецифическими нестероидными противовоспалительными (НПВП) препаратами и находящимися в группе повышенного риска развития желудочно-кишечных осложнений, прием препарата Пантонекс- DR 20 как превентивной меры образования язвы желудка и двенадцатиперстной кишки, вызванной применением НПВП, следует ограничить.
Следует провести оценку уровня риска с учетом индивидуальных факторов риска, включая возраст (старше 65 лет), анамнез образования язвы желудка или двенадцатиперсной кишки, а также желудочно-кишечных кровотечений.
Больные должны проконсультироваться с врачом в следующих случаях:
— если симптоматика не прошла в течение 2-х недель после непрерывной терапии
— непроизвольное снижение массы тела, анемия, желудочно-кишечное кровотечение, дисфагия, стойкая рвота или рвота с кровью, в этих случаях, следует исключать наличие злокачественного процесса
— ранее была язва желудка, или перенесенная хирургическая операция на желудке
— нарушение
пищеварения или изжога в течение 4 или более недель
— желтуха, нарушения функции или заболевания печени
— любое другое серьезное заболевание, влияющее на общее самочувствие
— пациенты старше 55 лет при наличии новых или недавно изменившихся симптомов.
Пантонекс- DR 20, как все препараты, блокирующие секрецию кислоты в желудке, может уменьшить всасывание витамина В12 (цианокобаламина) вследствие гипо- или ахлогидрии. Это следует учитывать при наличии соответствующих клинических симптомов.
При продолжительном периоде приема препарата, особенно свыше 1 года, пациенты должны находиться под регулярным наблюдением.
Понижение кислотности желудка повышает количество желудочных бактерий, которые обычно присутствуют в желудочно-кишечном тракте. Лечение препаратами, понижающими кислотность, приводит к повышению риска развития желудочно-кишечных инфекций, вызываемых такими микроорганизмами, как Salmonella, Campylobacter.
Особенности влияния лекарственного средства на способность управлять транспортным средством или потенциально опасными механизмами.
Следует воздержаться от управления транспортными средствами и другими механизмами, требующими повышенного внимания, из-за вероятности головокружений и нарушения зрения

Лечение: при подозрении на передозировку рекомендуется проведение поддерживающей и симптоматической терапии. Препарат не выводится посредством гемодиализа
Форма выпуска и упаковка
По 10 таблеток помещают в контурную ячейковую упаковку из фольги алюминиевой.
По 3 контурные упаковки вместе с инструкцией по медицинскому применению на государственном и русском языках вкладывают в пачку картонную.

Why it’s used
Panto-Denk 40 Tablet is a prescription medicine that is used for the short-term treatment of digestive diseases in which stomach acid and bile flows back and irritates the food-pipe, also known as GERD. This medicine works by suppressing the production of acid in the stomach. Panto-Denk 40 Tablet is also used for the management of inflammation and irritation of the food pipe, to treat the overproduction of stomach acid, and to prevent stomach ulcers caused by the use of certain medicines (called as, NSAIDs).
Proton Pump Inhibitors
Panto-Denk 40 belongs to the Proton Pump Inhibitors class of medicines. Proton-pump inhibitors (PPIs) are a group of medicines that significantly reduce stomach acid production. Excess acid production causes diseases related to the stomach and intestines. PPIs are used to treat such disorders.

How to use
Follow directions on the product label, information guide, and provided by your doctor before using Panto-Denk 40. Use this medicine as per your doctor’s instructions.
Panto-Denk 40 is used with or without food. You should swallow Panto-Denk 40 tablets whole. Do not split, crush or chew the tablets. Panto-Denk 40 is used 30 minutes before meals with applesauce (for delayed-release oral suspension).
Typical Dosage
The typical adult dose of Panto-Denk 40 is 40 mg once a day. The typical dose for children is 20 mg — 40 mg once a day. The maximum dose for adults of Panto-Denk 40 is 240 mg per day. This medicine is commonly used for a period of 8 weeks for erosive esophagitis associated with GERD. Panto-Denk 40 is typically used at the same time every day. It takes 2.5 hours for this medicine to start its action. There are no known habit-forming tendencies with this medicine.
Panto-Denk 40 is to be used for longer periods of time. Panto-Denk 40 may be prescribed for long-term treatment of hypersecretory conditions that produce excess stomach acid and for the long-term treatment of inflammation of the food pipe (erosive esophagitis). You should continue to use this medicine as directed by the doctor even if you feel well.
If using the liquid form of this medicine, measure the dose using the provided measuring cup, spoon, or dropper. Before pouring the medicine into the measuring device, you should check the measurement markings carefully. Then, pour the dose amount into the device. After use, clean and store the measuring device in a safe place for your next use. You should not use a tablespoon or teaspoon as the dose measuring devices since it can result in an incorrect dose. If indicated on the product package, shake the medicine before use.
If using the delayed-release form of this medicine, do not crush or chew the medicine, unless indicated on the package. Crushing or chewing of the medicine can result in unpleasant taste resulting in patients not following the medicine schedule. Crushing or chewing can also release all of the medicine at once, resulting in a decrease of effectiveness and a possible increase in side-effects.
The injection form of this medicine is for intravenous use only.
Talk to Your Doctor
Tell your doctor if your condition does not improve or worsens. Talk to your doctor if conditions like irregular and tremor, dizziness, abnormal or fast heartbeat, shaking movements of the body, muscle weakness, and muscle pain occurs. If you have issues with the health of your liver, a daily dose of 20 mg will usually be prescribed in patients having issues with their liver. Consult with your doctor before stopping the use of Panto-Denk 40 Tablet.
To see the impact of Panto-Denk 40 Tablet on the body, your doctor may recommend a lower initial dose. Please follow your doctor’s instructions carefully.
Use in Children
If you are giving Panto-Denk 40 Tablet to a child, be sure to use a product that is meant for use in children. Before giving this medicine, use the child’s weight or age to find the right dose from the leaflet or product package. You can also read the dosage section of this page to know the correct dose for your child. Else, consult with your doctor and follow their recommendation.
Lab Tests
Your doctor may require that certain tests be performed before using Panto-Denk 40 Tablet. The doctor may ask you to have Magnesium test. Magnesium levels in the body should be checked before starting the treatment because low magnesium can happen in individuals who take a proton pump inhibitor medicine after three months of treatment.
Medicines may be given for uses other than those listed in the medicine guide. Do not use Panto-Denk 40 Tablet for symptoms for which it was not prescribed. Do not give Panto-Denk 40 Tablet to others who may have similar symptoms as you. Self-medication can cause harm.

How to take Panto-Denk 40
Your dose and how often you take Panto-Denk 40 will depend on the following factors:
- age
- weight
- patient’s health
- the health of the patient’s kidneys
- medicines recommended by the doctor
- any other medicines being used
- herbal supplements in use
- response to the medicine
Panto-Denk 40 Dosage
Dosage for short-term treatment of GERD
Adult
- Recommended: 40 mg once daily for up to 8 weeks
Children (5 years and older having weight more than or equal to 40 kg/88 lb)
- Recommended: 40 mg
Children (5 years and older with weight between 15-40 kg/33-88 lb)
- Recommended: 20 mg once daily for up to 8 weeks
Dosage for the healing of inflammation of food pipe
Adult
- Recommended: 40 mg once daily
Dosage for overproduction of stomach acid (Zollinger-Ellison syndrome)
Adult
- Recommended: 40 mg twice daily (adjust dosage based on patient needs)
- Maximum: 240 mg per day

Special Instructions
Tablet
If you are not able to swallow a single 40 mg tablet, take two 20 mg tablets instead.
Delayed release oral suspension in applesauce
Prepare the granules in one teaspoonful (5 ml) of applesauce and do not use other food. Take this suspension within 10 minutes of preparation. Do not chew or crush the granules.
Suspension use through nose or stomach
Prepare the granules in two teaspoonfuls (10 ml) of apple juice and shake the preparation well. Repeat this procedure at least twice using the same method each time, and no granules should be present in the tube.
Delayed release oral suspension in applejuice
Prepare the granules in one teaspoonful (5 ml) of apple juice. Stir the preparation for 5 seconds and swallow immediately.
Powder for solution for injection
After preparation, use the solution within 12 hours.
Missed Dose
In case a dose is missed, take it as soon as you remember but if it is the time of the next dose then take the next regular dose. Two doses at the same time should not be taken to compensate for the missed dose.
Overdose
What to do if you overdose on Panto-Denk 40?
The treatment should depend upon the symptoms that appear (symptomatic and supportive treatment).

Precautions while using Panto-Denk 40
Before you use Panto-Denk 40, tell your doctor of your medical and health history including the following:
- disease in which body attacks its own tissues (also known as, systemic lupus erythematosus)
- low magnesium level
- low vitamin B12 level in the body
- stomach ulcers
- upper gastrointestinal bleeding
- weak bone density caused by osteoporosis
Before you use Panto-Denk 40 Tablet, tell your doctor if you are allergic to it or its ingredients. Your doctor may prescribe an alternative medicine and update your medical records to record this information. Tell your doctor if you are allergic to benzimidazole.
The use of this medicine may change the vitamin B12 level in the body. If you are taking any proton pump inhibitor over a period of more than three years, it may lead to the body not absorbing vitamin B12 properly.
The use of Panto-Denk 40 Tablet may change magnesium levels in the body. If you are taking any proton pump inhibitor for at least three months, this can lead to low level of magnesium in your body.
Use in Pregnancy
Consult with your doctor on the use of Panto-Denk 40 Tablet during pregnancy. It is not known if this medicine can harm the unborn baby. There have been no controlled studies in humans so its best to only use Panto-Denk 40 in pregnancy when required.
Use while Breastfeeding
Consult with your doctor on the use of Panto-Denk 40 during breastfeeding. This medicine may pass into breast milk but your doctor can best judge if it can impact the baby.
Impact on Fertility
Consult with your doctor on the use of Panto-Denk 40 Tablet, if you are trying to conceive. Impact of this medicine on fertility is not known although animal studies have shown no negative impact on fertility.
Seizures
Panto-Denk 40 Tablet can make you feel sleepy. Be careful while driving, using machinery, or doing any other activity that needs you to be alert. The consumption of alcohol with Panto-Denk 40 Tablet can make you feel more sleepy. Panto-Denk 40 Tablet may cause seizures in some people. Discuss with your doctor if you perform activities where a loss of consciousness may cause you or others harm.
Increased Risk
This medicine may increase your sensitivity to sunlight. If this happens, limit your time outdoors and prevent sunburns. Cover your skin or use a sunscreen.
Side-effects in Children
Children using this medicine may see an increased risk of vomiting, headache, rash, fever, stomach pain, diarrhea, and lung infection.

Warning: High doses of proton pump inhibitors (PPIs) taken over a long duration (>1 year), may increase the risk of wrist, spine, and hip fractures.
Panto-Denk 40 Side-effects
The following side-effects may commonly occur in children when using Panto-Denk 40 Tablet. Discuss with your doctor if any of these side-effects last for a long time or are severe:
- fever
- frequent passing of loose or watery stools (diarrhea)
- headache
- lung infection
- rash
- stomach pain
- vomiting
Rarely, the use of Panto-Denk 40 Tablet may cause the following side-effects:
- abnormal sensations in the body
- blurred vision
- burned skin layer (Lyell syndrome)
- burning sensation in body parts
- constipation
- dry mouth
- facial swelling
- fatigue
- feeling of discomfort
- fluid collection in body tissues
- fracture of the hip, wrist or spine
- gas or fluid, abdominal distension/bloating
- hallucination
- high lipid levels in the body
- increased creatine kinase level
- increased levels of fatty acids
- increased transaminases level
- inflammation of the liver (hepatitis)
- insomnia
- involuntary contraction of muscles
- kidney failure
- loss of body balance
- lupus
- mental confusion
- muscle pain
- physical weakness
- severe itching of the skin
- skin rash
- skin rash caused by disease or fever
- skin rashes with itchy and raised bumps
- stomach discomfort
- swelling due to fluid build up inside the veins and arteries
- swelling in-between kidney tubules (interstitial nephritis)
- swollen male breast tissue
- taste disorder
- weight changes
The following severe side-effects may also occur when using Panto-Denk 40 Tablet:

Blood and lymphatic system disorders
Symptoms: low levels of thrombocytes, deficiency of red blood cells, white blood cells and platelets

Symptoms: sensitivity to light, red coloured rashes on the skin swelling of the lower layer of skin

Low levels of magnesium
Symptoms: seizures, dizziness, abnormal or fast heartbeat, jerking or shaking movements of the body, muscle weakness, uncontrolled muscle contractions of the hands and feet, muscle ache involuntary muscle contractions of the voice box

Warnings
Increased Levels of Chromogranin A (protein)
This medicine increases chromogranin A level, which can interfere in the examination of neuroendocrine tumors. Panto-Denk 40 treatment should be discontinued for at least five days before the start of chromogranin A measurements.
Risk of Bacterial Infections
This medicine may lead to increased risk of infections caused by bacteria such as Salmonella and Campylobacter in the stomach or intestines.
Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)
Patients who are using Panto-Denk 40 to prevent stomach and intestinal issues because of treatment with NSAIDs should be evaluated. The evaluation should make sure that they need NSAIDs and have risk factors that may cause them to develop stomach and intestinal issues caused by NSAIDs.
Stomach Cancer
Patients with stomach cancer are at an increased risk when using this medicine. Effect of Panto-Denk 40 overlaps the symptoms of stomach cancer and may delay its diagnosis.
Abnormal Liver Function
Patients having issues with the functioning of their liver are at an increased risk when using this medicine. Such patients may see an increase in levels of liver enzymes. Liver enzymes should be monitored regularly by your doctor during long-term therapy of Panto-Denk 40. The patient should discontinue the use of this medicine if there is an increase in levels of liver enzymes.
Magnesium Deficiency
Patients undergoing treatment with proton pump inhibitors for at least three months and after a year of therapy are at increased risk. Such patients may see an increased risk of side effects which include muscular spasms, palpitation, and seizures. In such cases, the level of magnesium in the body should be monitored before the start of treatment with Panto-Denk 40.
Long-Term Therapy Using Panto-Denk 40
Patients who are on high-dose and follow the long-term therapy of proton pump inhibitors are at an increased risk. Such patients may see an increased risk for fractures of the hip, wrist, or spinal area. These patients should use the lowest effective dose for a shorter duration of time.
Vitamin B12 Deficiency
Patients who use acid-suppressing medicines for long periods of time are at an increased risk. These patients may develop a vitamin B12 deficiency.
Use of Panto-Denk 40 with Methotrexate
Patients undergoing treatment with both proton pump inhibitors and methotrexate are at an increased risk. Panto-Denk 40 may increase the level and effects of methotrexate on the body. Withdrawal of the proton pump inhibitor on a temporary basis may be considered in these patients.
Helicobacter Pylori (Bacteria) Positive Patients
Such patients are at an increased risk when using this medicine. These patients may suffer from an increased risk of swelling and pain in the stomach, also known as atrophic gastritis.

Antiretroviral Drugs
Panto-Denk 40 Tablet may interact with antiretroviral drugs (atazanavir or nelfinavir), which are used to treat HIV infections. The combined use of these medicines reduces the level of HIV medicines in the body. This may lead to the loss of effects of the HIV medicines and development of resistance to the medicines.
Coumarin Anticoagulants
There may be an interaction of Panto-Denk 40 with coumarin anticoagulants (warfarin and phenprocoumon), which are used to prolong the time needed for blood to clot. Abnormal bleeding occurs when both the medicines are used together which can be possibly fatal. Patients undergoing treatment with both the drugs should be closely monitored for an increase in bleeding time.
Antimetabolites
Panto-Denk 40 Tablet interacts with methotrexate, which is used to treat rheumatoid arthritis, itchy, psoriasis, and cancer. The combined use of these medicines may lead to increased level and effects of methotrexate on the body. In such cases, the temporary withdrawal of Panto-Denk 40 may be required.
Azole Antifungals
Special instructions need to be followed while taking this medicine along with azole antifungals (ketoconazole, itraconazole, and posaconazole) which are used to treat fungal infections. Panto-Denk 40 may interfere with absorption of these medicines.
Urine Screening Tests
Your doctor’s guidelines may need to be followed while taking this medicine along with Urine tests. False positive urine screening tests for certain chemicals (such as, tetrahydrocannabinol) has been reported in patients using Panto-Denk 40. Panto-Denk
Panto-Denk is chemically a novel substituted benzimidazole derivative, which suppresses the final step in gastric acid production by forming a covalent bond to two sites of H+/K+ATPase enzyme system at the secretory surface of the gastric parietal cell. This leads to inhibition of both basal and stimulated gastric effect that persists longer than 24 hours.
Panto-Denk is quantitatively absorbed and its bioavailability does not change upon multiple dosing. Panto-Denk is extensively metabolized in the liver. Almost 80% of an oral dose is excreted as metabolites in urine; the remainder is found in feces.
This drug acts to decrease gastric acid secretion, which reduces stomach acidity. Panto-Denk administration leads to long-lasting inhibition of gastric acid secretion.
General Effects
Panto-Denk has been shown to reduce acid reflux-related symptoms, heal inflammation of the esophagus, and improve patient quality of life more effectively than histamine-2 receptor antagonists (H2 blockers). This drug has an excellent safety profile and a low incidence of drug interactions. It can be used safely in various high-risk patient populations, including the elderly and those with renal failure or moderate hepatic dysfunction.
How Panto-Denk works
Hydrochloric acid (HCl) secretion into the gastric lumen is a process regulated mainly by the H(+)/K(+)-ATPase of the proton pump, expressed in high quantities by the parietal cells of the stomach. ATPase is an enzyme on the parietal cell membrane that facilitates hydrogen and potassium exchange through the cell, which normally results in the extrusion of potassium and formation of HCl (gastric acid).
Proton pump inhibitors such as pantoprazole are substituted benzimidazole derivatives, weak bases, which accumulate in the acidic space of the parietal cell before being converted in the canaliculi (small canal) of the gastric parietal cell, an acidic environment, to active sulfenamide derivatives. This active form then makes disulfide bonds with important cysteines on the gastric acid pump, inhibiting its function. Specifically, pantoprazole binds to the sulfhydryl group of H+, K+-ATPase, which is an enzyme implicated in accelerating the final step in the acid secretion pathway. The enzyme is inactivated, inhibiting gastric acid secretion. The inhibition of gastric acid secretion is stronger with proton pump inhibitors such as pantoprazole and lasts longer than with the H(2) antagonists.

Dosage
Panto-Denk dosage
Tablet:
The usual recommended adult oral dose is 40 mg given once daily, preferably in the morning with or without food. The duration of therapy is ranging from 2-8 weeks.
- Duodenal ulcers: Panto-Denk 40 mg tablet once daily for 2-4 weeks.
- Gastric ulcer: Panto-Denk 40 mg tablet once daily for 4-8 weeks.
- Reflux esophagitis: Panto-Denk 40 mg tablet once daily for 4-8 weeks.
- Ulcers induced by NSAIDs: Panto-Denk 40 mg tablet once daily.
- Maintenance therapy: Maintenance therapy should involve the lowest effective dose of the drug. Panto-Denk both 20 mg & 40 mg doses are safe and effective in maintaining patients with healed reflux esophagitis and PUD in remission.
IV Injection:
- Duodenal ulcer and gastric ulcer: 40 mg once daily for 7-10 days
- Gastroesophageal reflux disease associated with a history of erosive esophagitis: 40 mg once daily for 7-10 days
- Prevention of rebleeding in peptic ulcer: IV 80 mg, followed by 8 mg/hour infusion for 72 hours
- Prophylaxis of acid aspiration: 80 mg IV every 12 h for 24 h, followed by 40mg every 12 h
- Long-term management of Zollinger-Ellison Syndrome and other pathological hypersecretory conditions: 80 mg IV every 12 hours, may increase to 80 mg every 8 hours if needed, may titrate to higher doses depending on acid output.
DIRECTION FOR USE OF IV INJECTION: Panto-Denk lyophilized powder and 0.9% Sodium Chloride Injection is for intravenous administration only and must not be given by any other route. Panto-Denk IV injection should be given as a slow intravenous injection. The solution for IV injection is obtained by adding 10 ml 0.9% Sodium Chloride Injection to the vial containing powder. After reconstitution the injection should be given slowly over a period of at least 2 to 5 minutes. Use only freshly prepared solution. The reconstituted solution may be stored at room temperature (up to 30° C) for a maximum 4 hours.
DIRECTION FOR USE OF IV INFUSION: Panto-Denk IV infusion should be given as an intravenous infusion over a period of approximately 15 minutes. Panto-Denk IV infusion should be reconstituted with 10 ml of 0.9% Sodium Chloride Injection and further diluted (admixed) with 0.9% Sodium Chloride Injection or 5% Dextrose or Lactated Ringer’s Injection to a final volume of 100 ml. The reconstituted solution may be stored at room temperature (up to 30° C) for a maximum 4 hours prior to further dilution. The admixed solution may be stored at room temperature (up to 30° C) and must be used within 24 hours from the time of initial reconstitution.
Side Effects
No potentially life-threatening effects have been reported with Panto-Denk. Symptomatic adverse effects include headache and diarrhoea are two common reported adverse effects. Peripheral edema has been occasionally reported in female patients. Other side effects may include abdominal pain, dizziness, nausea, epigastric discomfort, flatulence, skin rash, pruritus etc.

Tumorigenicity
Because of the chronic nature of GERD, there may be a potential for long-term administration of pantoprazole. In long-term rodent studies, pantoprazole was carcinogenic and its administration lead to rare types of gastrointestinal tumors. The relevance of these findings to tumor development in humans is unknown at this time.
Teratogenic Effects
This drug falls under pregnancy category B category. Reproduction studies have been performed in rats at oral doses up to 88 times the recommended human dose (RHD), as well as in rabbits at oral doses up to 16 times the RHD, and have shown no evidence of impaired fertility or harm to the fetus caused by pantoprazole. No adequate and well-controlled studies in pregnant women have been completed. Because animal reproduction studies are not always predictive of human response, this drug should only be used during pregnancy if clearly required.
Nursing Mothers
Panto-Denk and its metabolites have been found to be excreted in the milk of rats. Panto-Denk excretion in human milk has been found in a study performed with a single nursing mother after one 40 mg oral dose. The clinical relevance of this finding is not known, however, it is advisable to take note of this finding when considering pantoprazole use during nursing. Many drugs excreted in human breastmilk have a risk for serious adverse effects in nursing infants.
Precaution
Patients should be cautioned that Panto-Denk tablet should not be split, crushed or chewed. The tablet should be swallowed whole, with or without food in the stomach. Concomitant administration of antacid does not affect the absorption of Panto-Denk.
Interaction
There is no interaction with concomitantly administered antacids. No dosage adjustment is needed with combination use of the following drugs: Theophylline, Caffeine, Diazepam, Digoxin, Ethanol, Metoprolol, Nifedipine or Warfarin.

Volume of Distribution
The apparent volume of distribution of pantoprazole is approximately 11.0-23.6 L, distributing mainly in the extracellular fluid.
Elimination Route
Panto-Denk is absorbed after oral administration as an enteric-coated tablet with maximum plasma concentrations attained within 2 – 3 hours and a bioavailability of 77% that does not change with multiple dosing . Following an oral dose of 40mg, the Cmax is approximately 2.5 μg/mL with a tmax of 2 to 3 hours. The AUC is approximately 5 μg.h/mL. There is no food effect on AUC (bioavailability) and Cmax.
Delayed-release tablets are prepared as enteric-coated tablets so that absorption of pantoprazole begins only after the tablet leaves the stomach.
Clearance
Adults: With intravenous administration of pantoprazole to extensive metabolizers, total clearance is 7.6-14.0 L/h.
In a population pharmacokinetic analysis, the total clearance increased with increasing body weight in a non-linear fashion.
Children: clearance values in the children 1 to 5 years old with endoscopically proven GERD had a median value of 2.4 L/h.
Elimination Route
After a single oral or intravenous (IV) dose of 14C-labeled pantoprazole to healthy, normal metabolizing subjects, about 71% of the dose was excreted in the urine, with 18% excreted in the feces by biliary excretion. There was no kidney excretion of unchanged pantoprazole.
Pregnancy & Breastfeeding use
There are no adequate or well-controlled studies in pregnant women. Panto-Denk should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
It is not known whether Panto-Denk is excreted in human breast milk. Panto-Denk should be used during lactation only if the potential benefit justifies the potential risk.
Contraindication
It is contraindicated in patients with known hypersensitivity to Panto-Denk.
Acute Overdose
There are no known symptoms of overdosage in humans. Since Panto-Denk is highly protein bound, it is not readily dialyzable. Apart from symptomatic and supportive management, no specific therapy is recommended.

Механизм действия
Пантопразол является замещенным бензимидазолом, который ингибирует секрецию соляной кислоты в желудке путем специфической блокады протонной помпы париетальных клеток. Пантопразол преобразуется в свою активную форму, циклический сульфенамид, в кислотной среде в париетальных клетках, где он ингибирует фермент Н+, К+-АТФазу, т.е. заключительную стадию образования соляной кислоты в желудке.
Ингибирование зависит от дозы и влияет как на базальную, так и на стимулированную секрецию кислоты. У большинства пациентов купирование симптомов изжоги и кислотного рефлюкса достигается в течение двух недель. Пантопразол уменьшает кислотность в желудке и тем самым увеличивает уровень гастрина пропорционально снижению кислотности. Повышение содержания гастрина имеет обратимый характер. Т.к. препарат связывается с ферментом, дистальным по отношению к рецептору, он может ингибировать секрецию соляной кислоты независимо от стимуляции другими веществами (ацетилхолин, гистамин, гастрин). Эффект одинаков независимо от того, принимается активное вещество перорально или внутривенно.

В ретроспективном анализе 17 исследований на 5960 пациентах с гастроэзофагеальным рефлюксом, которые получали лечение пантопразолом 20 мг, была проведена оценка симптомов, связанных с кислотным рефлюксом (изжога, кислотная регургитация), в соответствии со стандартизированной методологией. Выбранные для исследования случаи должны были иметь хотя бы одну запись о симптоме кислотного рефлюкса за две недели. Диагноз гастроэзофагеальный рефлюкс в данных исследованиях основывался на эндоскопической оценке, за исключением одного исследования, в котором отбор пациентов проводился исключительно на симптоматологии. В данных исследованиях процент пациентов, полностью избавившихся от изжоги через 7 дней, составил 54,0-80,6% в группе, принимавшей пантопразол. Через 14 и 28 дней полное избавление от изжоги было установлено у 62,9-88,6% и 68,1-92,3% пациентов соответственно.
Для полного избавления от кислотной регургитации были получены похожие результаты, как в случае с изжогой. Через 7 дней процент пациентов, полностью избавившихся от кислотной регургитации, составил 61,5-84,4%, через 14 дней – 67,7-90,4% и через 28 дней – 75,2-94,5%.
Пантопразол стабильно показывал лучшую эффективность, чем антагонисты Н2-рецепторов, и не хуже, чем другие ингибиторы протонной помпы. Облегчение симптомов кислотного рефлюкса в значительной степени зависело от исходной стадии гастроэзофагеального рефлюкса.
– гастроэзофагеальная рефлюксная болезнь, рефлюкс-эзофагит;
– эрадикация Helicobacter pylori (в составе комплексной терапии);
– язва двенадцатиперстной кишки;
– язва желудка;
– синдром Золлингера-Эллисона и другие патологические гиперсекреторные заболевания.
Внутрь, до еды (обычно перед завтраком), запивая необходимым количеством воды. Таблетки следует проглатывать целиком, не разжевывая.
Взрослые и подростки в возрасте от 12 лет
Лечение гастроэзофагеальной рефлюксной болезни, рефлюкс-эзофагита:
По 1 таблетке пантопразола в день. В отдельных случаях дозу можно увеличить до 2 таблеток пантопразола в день. Обычно требуется 4 недели для лечения рефлюксного воспаления пищевода. При необходимости продолжительность лечения увеличивают до 8 недель.
Взрослые
Эрадикация Helicobacter pylori (в комплексной терапии):
У Helicobacter pylori-положительных пациентов с язвами желудка и двенадцатиперстной кишки следует достигать эрадикации микроорганизма с помощью комбинированной терапии.
Следует ориентироваться на официальные местные руководства (например, национальные рекомендации) в вопросах бактериальной резистентности и соответствующего назначения и применения антибактериальных средств. В зависимости от спектра резистентности, следующие комбинации могут быть рекомендованы:
a. Одна таблетка пантопрозола дважды в сутки
— 1000 мг амоксициллина дважды в сутки
— 500 мг кларитромицина дважды в сутки
b. Одна таблетка пантопразола дважды в сутки
— 400-500 мг метронидазола (или 500 мг тинидазола) дважды в сутки
— 250-500 мг кларитромицина дважды в сутки
c. Одна таблетка пантопразола дважды в сутки
— 1000 мг амоксицилина дважды в сутки
— 400-500 мг метронидазола (или 500 мг тинидазола) дважды в сутки.
При проведении эрадикационной терапии вторая таблетка пантопразола должна быть принята за 1 час до вечернего приема пищи. Комбинированная терапия проводится обычно в течение 7 дней и может быть продолжена еще на 7 дней при общей продолжительности 14 дней. Для обеспечения заживления язв затем показана дальнейшая терапия пантопразолом с использованием доз, рекомендованных для язв двенадцатиперстной кишки и желудка.
Если комбинированная терапия не показана (Helicobacter pylori-отрицательный пациент), следует руководствоваться следующими дозовыми режимами для монотерапии пантопразолом.
Лечение язвы желудка:
По 1 таблетке пантопразола в день. В отдельных случаях дозу можно увеличить до 2 таблеток пантопразола в день. Обычно требуется 4 недели для лечения язвы желудка. При необходимости продолжительность лечения увеличивают до 8 недель.
Лечение язвы двенадцатиперстной кишки:
По 1 таблетке пантопразола в день. В отдельных случаях дозу можно увеличить до 2 таблеток пантопразола в день. Обычно требуется 2 недели для лечения язвы двенадцатиперстной кишки. При необходимости продолжительность лечения увеличивают до 4 недель.
Синдром Золлингера—Эллисона и другие патологические гиперсекреторные заболевания:
Для длительного лечения синдрома Золлингера-Эллисона и других патологических гиперсекреторных заболеваний, лечение начинают с ежедневной дозы 80 мг (2 таблетки пантопразола по 40 мг). После чего дозу можно повышать или понижать по мере необходимости, руководствуясь показателями секреции кислоты желудочного сока. При ежедневной дозировке свыше 80 мг, дозу необходимо разделить и применять дважды в день. Возможно временное увеличение дозировки более 160 мг, но только на время, необходимое для адекватного подавления секреции соляной кислоты. Длительность лечения синдрома Золлингера-Эллисона и других патологических гиперсекреторных заболеваний не ограничена и может проводиться в соответствии с клинической необходимостью.
Дети в возрасте до 12 лет:
Пантопразол не рекомендован для применения у детей младше 12 лет в связи с ограниченными данными по безопасности и эффективности в данной возрастной группе.
Особые группы пациентов
Пациенты с нарушением функции печени
У пациентов с печеночной недостаточностью тяжелой степени не следует превышать суточную дозу пантопразола 20 мг (следует использовать другие лекарственные средства с дозировкой 20 мг). В процессе лечения пантопразолом необходимо следить за уровнем ферментов печени в плазме крови у таких пациентов. В случае повышения активности ферментов печени необходимо прекратить прием пантопразола. Пантопразол не следует применять в составе комбинированной терапии для эрадикации Helicobacter pylori у пациентов с умеренной или тяжелой печеночной недостаточностью, поскольку в настоящее время отсутствуют данные по эффективности и безопасности применения пантопразола в составе комбинированной терапии у таких пациентов.
Пациенты с нарушением функции почек
У пациентов с нарушением функции почек коррекции дозы не требуется. Препарат не следует применять в составе комбинированной терапии для эрадикации Helicobacter pylori с нарушением функции почек, поскольку в настоящее время отсутствуют данные по эффективности и безопасности пантопразола в составе комбинированной терапии у таких пациентов.
Пожилые пациенты
Коррекция дозы не требуется.
Приблизительно у 5% пациентов могут проявиться побочные эффекты препарата.
Наиболее частые побочные эффекты: диарея и головная боль – проявляются приблизительно у 1% пациентов.
Частота развития побочных эффектов приведена в следующей градации: очень часто (≥1/10); часто (≥1/100, <1/10); нечасто (≥1/1000, <1/100); редко (≥1/10000, <1/1000); очень редко (<1/10000); частота неизвестна (по имеющимся данным определить частоту встречаемости не представляется возможным).
Были зафиксированы следующие нежелательные побочные эффекты пантопразола:
Со стороны органов кроветворения: редко – агранулоцитоз. Очень редко – тромбоцитопения, лейкопения, панцитопения.
Со стороны нервной системы и органов чувств: нечасто – головная боль, головокружение, нарушение сна. Редко – нарушение остроты зрения, депрессия (и аггравация). Очень редко – дезориентация, парестезия. Частота неизвестна – галлюцинации, спутанность сознания (в особенности у предрасположенных пациентов, а также обострение данных симптомов в случае их наличия ранее).
Со стороны желудочно-кишечного тракта: часто – полипы желудка (доброкачественные). Нечасто – диарея, тошнота/ рвота, вздутие живота и метеоризм, запор, сухость во рту, боль и дискомфорт в животе.
Со стороны почек и мочевыводящих путей: частота неизвестна – интерстициальный нефрит (с возможным прогрессированием до почечной недостаточности).
Со стороны кожи и подкожных тканей: нечасто – сыпь/экзантема, зуд. Редко – крапивница, отек Квинке. Частота неизвестна: синдром Стивенса-Джонсона, синдром Лайелла, мультиформная эритема, фотосенсибилизация, подострая кожная красная волчанка.
Со стороны скелетно-мышечной и соединительной ткани: редко – артралгия, миалгия.
Со стороны обмена веществ и водно-электролитного обмена: редко – гиперлипидемия и повышение уровня жиров (триглицериды, холестерол), изменение массы тела. Частота неизвестна – гипонатриемия, гипомагниемия, гипокальциемия, гипокалиемия.
Со стороны иммунной системы: редко – гиперчувствительность (включая анафилактические реакции и анафилактический шок).
Со стороны печени и желчевыводящих путей: нечасто – повышение уровня ферментов печени (трансаминаз, γ-GT). Редко – повышение уровня билирубина. Частота неизвестна – повреждение гепатоцитов, желтуха, гепатоцеллюлярная недостаточность.
Со стороны половых органов и молочной железы: редко – гинекомастия.
Прочие: нечасто — астения, дисфория, утомляемость. Редко – повышение температуры тела, периферический отек.

Вы также можете сообщить о нежелательных реакциях в информационную базу данных по нежелательным реакциям (действиям) на лекарственные препараты, включая сообщения о неэффективности лекарственных препаратов, выявленным на территории государства (УП «Центр экспертиз и испытаний в здравоохранении М3 РБ», rceth.by). Гиперчувствительность к пантопразолу, производным бензимидазолов или другим компонентам препарата. Одновременный прием атазанавира. Из-за недостаточного количества данных не рекомендуется использование пантопразола для лечения детей до 12 лет.
С осторожностью: беременность, период лактации, печеночная недостаточность.
До настоящего времени явлений передозировки в результате применения препарата пантопразол отмечено не было. Дозы до 240 мг, применяемые внутривенно в течение 2 минут, переносились хорошо. Поскольку пантопразол в значительной степени связывается с протеинами плазмы крови, он плохо подвергается гемодиализу. В случае передозировки с клиническими признаками интоксикации, помимо симптоматического и поддерживающего лечения, каких-либо отдельных терапевтических рекомендаций не имеется.
Злокачественное новообразование желудка
При наличии любого настораживающего симптома (например, существенного снижения массы тела, повторной рвоты, дисфагии, рвоты кровью, анемии, мелены) и при наличии или подозрении на язву желудка следует исключить злокачественное новообразование, поскольку лечение пантопразолом может ослаблять симптомы и отсрочить постановку диагноза.
Следует рассмотреть необходимость дополнительного обследования при сохранении симптоматики, несмотря на проведение адекватной терапии.
Совместное применение с ингибиторами протеазы ВИЧ
Не рекомендуется совместной применение пантопразола с ингибиторами протеазы ВИЧ, всасывание которых зависит от кислотности pH желудка, таких как атазанавир, в связи со значительным снижением их биодоступности.
Влияние на всасывание витамина В12
У пациентов с синдромом Золлингера-Эллисона и другими патологическими гиперсекреторными состояниями, требующими длительной терапии, пантопразол, как все антисекреторные средства, могут снижать всасывание витамина В12 (цианокобаламина) вследствие гипо- или ахлоргидрии. Это следует учитывать у пациентов со сниженными запасами данного витамина или при наличии факторов риска для снижения его всасывания при длительной терапии или при наличии соответствующих клинических симптомов.
Длительное лечение
При проведении длительной терапии, в особенности, когда она превышает период одного года, пациенты должны находиться под регулярным наблюдением.
Желудочно-кишечные инфекции бактериальной природы
Лечение пантопразолом может приводить к слегка повышенному риску желудочно-кишечных инфекций, вызываемых такими бактериями, как Salmonella и Campylobacter или С. Difficile.
Гипомагниемия
Сообщалось о выраженной гипомагниемии у пациентов, леченных ингибиторами протонного насоса в течение как минимум трех месяцев (в большинстве случаев – в течение года). Могут возникать серьезные проявления гипомагниемии, такие как утомляемость, тетания, делирий, судороги, головокружение и желудочковая аритмия, однако они могут начинаться скрыто и могут оказаться незамеченными. У большинства пациентов гипомагниемия купировалась введением магния на фоне отмены ингибиторов протонного насоса.
Для пациентов, которым предполагается проведение длительного лечения или принимающим ингибиторы протонного насоса совместно с дигоксином или лекарствами, которые могут вызывать гипомагниемию (например, диуретиками), медицинские работники должны рассмотреть необходимость измерения уровня магния в крови до начала лечения и периодически в период его проведения.
Переломы костей
Ингибиторы протонного насоса, особенно при использовании в высоких дозах и в течение длительного времени (более одного года) могут умеренно повышать риск переломов бедра, запястья и позвоночника, в основном у пожилых или при наличии других признанных факторов риска. Наблюдательные исследования указывают, что ингибиторы протонного насоса могут повышать общий риск переломов на 10-40%.Определенная часть этого повышения может быть связана с другими факторами риска. Пациенты с риском остеопороза должны получать медицинскую помощь в соответствии с современными клиническими руководствами и потреблять достаточное количество витамина D и кальция.
Подострая кожная красная волчанка (ПККВ)
Применение ингибиторов протонного насоса связано с очень редкими случаями ПККВ. В случае появления патологических изменений, особенно на участках кожи, подвергшихся солнечному воздействию, и в случае присоединения артралгии, пациент должен незамедлительно обратиться за медицинской помощью, а медицинский работник должен оценить целесообразность отмены препарата. ПККВ после предшествующего лечения ингибитором протонного насоса может повышать риск ПККВ при применении других ингибиторов протонного насоса.
Искажение лабораторных показателей
Повышенный уровень CgA может искажать результаты исследований на предмет выявления нейроэндокринных опухолей. Чтобы избежать интерференции, лечение препаратом пантопразол должно быть прекращено по крайней мере за 5 дней до определения уровня CgA (см. раздел «Фармакодинамика»). Если уровни CgA и гастрина не вернулись в диапазон нормальных значений после начального определения, измерение должно быть проведено повторно через 14 дней после прекращения терапии ингибиторами протонного насоса.
Опыт применения пантопразола у беременных женщин очень ограничен. В исследованиях репродуктивности на животных наблюдались признаки небольшой фетотоксичности. Доклинические исследования не выявили признаков снижения фертильности или каких-либо тератогенных эффектов. Потенциальный риск применения препарата у человека остается неизвестным. Данное лекарственное средство не рекомендуется использовать во время беременности.
Исследования на животных продемонстрировали, что пантопразол попадает в грудное молоко. Сообщалось о попадании в грудное молоко человека. Риск для плода/новорожденного не может быть исключен, поэтому необходимо прервать кормление грудью при лечении пантопразолом.
В связи с риском развития побочных реакций во время лечения препаратом, необходимо воздерживаться от вождения автотранспорта и занятий видами деятельности, требующими повышенной концентрации внимания.
Лекарственные средства с всасыванием, зависящим от pH желудка
Пантопразол может снизить всасывание активных веществ, био доступность которых зависит от уровня pH в желудке (например, соли железа, кетоконазол).
Ингибиторы протеазы ВИЧ
Было продемонстрировано, что одновременное применение атазанавира 300 мг или ритонавира 100 мг с омепразолом (40 мг раз в день) или атазанавира 400 мг с лансопразолом (60 мг однократная доза) здоровыми добровольцами привело к существенному снижению биодоступности атазанавира. Всасывание атазанавира зависит от уровня pH. Поэтому пантопразол нельзя применять одновременно с атазанавиром.
Кумариновые антикоагулянты (варфарин)
Совместное применение пантопразола с варфарином не влияло на фармакокинетику или международное нормализованное отношение (МНО) варфарина. Однако сообщалось об увеличении МНО и протромбинового времени у пациентов, получавших другие ингибиторы протонного насоса и варфарин одновременно. Увеличение МНО и протромбинового времени может приводить к кровотечению и даже к смерти. У пациентов, получающих пантопразол и варфарин, может потребоваться мониторинг на предмет повышения МНО и протромбинового времени.
Метотрексат
Сообщалось, что совместное применение высокой дозы метротрексата (например, 300 мг) и ингибиторов протонного насоса повышает уровень метотрексата у некоторых пациентов. Таким образом, в условиях применения высоких доз метотрексата, например, при раке и псориазе, может быть рассмотрена временная отмена пантопразола.
Другие исследования взаимодействия
Пантопразол метаболизируется в печени с помощью ферментной системы цитохрома Р450. Основным метаболическим путем является деметилирование с участием CYP2C19, другие метаболические пути включают окисление с участием CYP3A4. Ингибиторы CYP2C19, такие как флувоксамин, могут повышать системное воздействие пантопразола. Может потребоваться снижение дозы у пациентов, которые длительное время получают высокие дозы пантопразола, или у тех, у кого имеется печеночная недостаточность. Индукторы ферментов, влияющие на CYP2C19 и CYP3A4, такие как рифампицин и зверобой (Hypericum perforatum), могут снижать плазменные концентрации ингибиторов протонного насоса, подвергающихся метаболизму посредством этих ферментных систем. Нельзя исключить взаимодействие пантопразола с другими веществами, которые метаболизируются той же ферментной системой. Однако не наблюдалось каких-либо клинически значимых взаимодействий в отдельных исследованиях с карбамазепином, кофеином, диазепамом, диклофенаком, дигоксином, этанолом, глибенкламидом, метопрололом, напроксеном, нифедипином, фенитоином, пироксикамом, теофиллином и оральными контрацептивами, содержащими левоноргестрел и этинилэстрадиол.
Взаимодействие с одновременно принимаемыми антацидами не наблюдалось.
Хранить в защищенном от влаги и света месте при температуре не выше 25 °C.
Хранить в недоступном для детей месте.
2 года.
Не использовать по истечении срока годности, указанного на упаковке.
По 14 или 28 таблеток в банках полимерных с крышкой натягиваемой с контролем первого вскрытия. Одну банку с инструкцией по применению помещают в пачку из картона коробочного.
