Contraindications: Contraindications exist by patients with malignant tumour wounds (non enteric fistulas) which have not yet been examined, osteomyelitis or necrotic tissue. The ATMOS ® S 042 NPWT must not be applied to exposed nerves, anastomotic areas, blood vessels or organs. The negative pressure device must not be used: — for non-medical applications.

3.3.4 Connecting/removing the vacuum unit with the wound dressing set The ATMOS negative pressuresystem has a double lumen suction hose system to prevent blocking of the suction hose. Ensure that the connections are always correctly connected in order to avoid faults.

Removal • Press the release button of the hose connector laterally and keep it pressed. • Now pull the two ends apart. • Prior to disposal break off the sealing plugs on the connector and plug it into the secretion hose on the device. This will ensure that the secretion cannot leak from the canister.

Attaching the holder clamp • Turn the turning knob of the holder clamp counter-clock- wise until the clamp can be attached to the desired fixture. • Turn the turning knob clockwise to fix the holder clamp. Ensure that the stand holder is firmly attached to the desired fixture. 3.3.6 Attaching/removing the vacuum unit at/from the universal bracket Firmly hold the vacuum unit during the whole process. • Place the vacuum unit onto the stand holder. Ensure that the thread at the bottom of the vacuum unit is directly above the fixing screw of the stand holder.

4.2.3 Secretion canister full The following causes can trigger the error message: • Secretion canister full: Exchange the secretion canister. • Blockage of the bacterial filter in the secretion canister: If the bacterial filter has become moist with secretion, it becomes blocked. Exchange the secretion canister. By touching the key this warning message can be sup- pressed for 5 minutes. In order to prevent blockage of the bacterial filter make sure that the negative pressure unit is set up in stable position.

Введение
В программе комплексного лечения в настоящее время широко используются методы физического высокотехнологического воздействия на ткани ран. В лечебной практике применяется следующее современное оборудование: ультразвуковая низкочастотная кавитация — аппарат SONOCA, плазменный поток — аппарат ПЛАЗОН, пульсирующая струя — аппарат Гидроджет, отрицательное давление — аппарат VivanoTec/S042 [1-3]. Данные методы позволяют быстро удалять нежизнеспособные ткани, малотравматичны, радикально уменьшают микробную обсемененность тканей.
Материал и методы
С ноября 2012 г. в отделении гнойной хирургии Центральной клинической больницы с поликлиникой УДП РФ для лечения ран, осложненных хирургической инфекцией, используют аппарат отрицательного давления VivanoTec/S042.
Этот метод физического высокотехнологического воздействия на ткани ран использован в лечении 74 больных. Преобладали женщины (73%) в возрасте от 18 до 89 лет, из них 87% имели тяжелые сопутствующие заболевания (ишемическую болезнь сердца, гипертоническую болезнь, хронические обструктивные заболевания легких, онкологические заболевания, сахарный диабет).
По характеру основного заболевания больные распределились следующим образом:
1) синдром диабетической стопы — 18 больных;
2) обширные пролежни туловища и конечностей — 16 больных;
3) нагноившиеся лапаротомные и торакальные раны — 14 больных (рис. 1);
4) посттравматический остеомиелит трубчатых костей конечностей — 17 больных;
5) послеоперационный остеомиелит грудины, гнойный медиастенит — 6 больных (рис. 2);
6) инфицированные эндопротезы крупных суставов, с развитием послеоперационного остеомиелита — 7 больных (рис. 3).
Степень тяжести состояния больных и органных дисфункций определяли по шкалам SAPS и SOFA. С помощью скрининг-исследования при поступлении у всех больных было выявлено не менее 1-2 признаков синдрома системной воспалительной реакции (CCВР), а у 3% больных диагностирован тяжелый сепсис (3-4 признака синдрома системной воспалительной реакции, наличие обширного очага поражения, результат прокальцитонинового теста >10 нг/мл).
При микробиологическом исследовании ран отмечали наличие полимикробной аэробной и анаэробной микрофлоры (S. pyogenes, S. aureus, B. fragilis, Peptostreptococcus, Ps. aeruginosa и др.) с уровнем контаминации выше критического — 106-108 КОЕ/мл.
Программа комплексного лечения включала:
1) радикальную хирургическую обработку гнойного очага с расширенной некрэктомией в пределах здоровых тканей;
2)коррекцию метаболических расстройств, компенсацию углеводного обмена в случае выраженных нарушений у пациентов с тяжелым диабетом в отделении реанимации;
3) использование принципа стартовой деэскалационной антибиотикотерапии препаратами ультраширокого спектра действия, с последующим переходом на этиотропную терапию.
В местном лечении раневого процесса применялись современные перевязочные материалы на основе карбоксиметилцеллюлозы с серебром, гидроколлоидные повязки, иммобилизированные протеолитические ферменты.
В программе комплексного лечения широко использовали методы физического высокотехнологического воздействия на ткани ран. Они включали применение отрицательного давления (аппарат VivanoTec/S042). Данный метод малотравматичен, позволяет быстро удалить нежизнеспособные ткани, радикально уменьшить микробную обсемененность тканей (при контрольных микробиологических исследованиях ран отмечали снижение уровня контаминации до 102-104 КОЕ/мл на 2-3-и сутки от начала лечения).
Спектр хирургических вмешательств зависел от фазы воспалительного процесса и выраженности клинических проявлений. Всего было выполнено 153 оперативных вмешательства. Выполнялась хирургическая обработка инфицированных ран, некрэктомия. По мере купирования воспалительного процесса в ране проводилась повторная хирургическая обработка раны с пластикой местными тканями.
Реконструктивные операции выполнялись только после регресса острых признаков воспаления, стабилизации общего состояния больного, нормализации показателей гомеостаза. Как правило, операции производили на 5-7-е сутки после первичной хирургической обработки.
Результаты
Продолжительность стационарного лечения варьировала от 10 до 38 дней. Хорошие ближайшие анатомические результаты достигнуты у 97% пролеченных пациентов. У всех больных был достигнут хороший функциональный результат лечения, заживление ран наступало в оптимальные сроки, значительно сократились сроки лечения.
Таким образом, в комплексном лечении ран, осложненных хирургической инфекцией, целесообразно использовать различные виды активного хирургического, общесоматического, антибактериального лечения, что позволит значительно снизить сроки купирования хирургической инфекции и оптимизировать длительность заживления раны. Использование в лечении методов физического высокотехнологического воздействия на ткани ран привело к существенному уменьшению числа инфекционных осложнений и функциональных расстройств, а также значительно сократило сроки лечения, с хорошими отдаленными анатомическими и функциональными результатами.
