Flogocid cream is composed of three main ingredients; Neomycin, Nystatin and Bufexamac. It is used to treat fungal skin infections. Офлокаин-Дарница® представляет собой комбинацию противомикробного средства группы фторхинолонов офлоксацина, и местного анестетика – лидокаина – на гидрофильной, гиперосмолярной и водорастворимой основе. Фармакологические эффекты мази обусловлены свойствами действующих веществ.Офлоксацин действует на бактериальный эффект за счет дестабилизации цепей ДНК бактерий.
Офлоксацин имеет широкий спектр действия: он активен по отношению к большинству грамотрицательных бактерий – Escherichia coli, Klebsiella spp. (включая Klebsiella сдщфсфу, Klebsiella pneumonia), Proteus spp. (включая Proteus mirabili, Proteus vulgaris), Morganella morganii, Shigella spp. (включая Shigella sonnei), Salmonella spp., Campylobacter jejuni, Aeromonas hydrophila, Vibrio cholera, Vibrio parahemolyticus, Haemophilus influenza, Haemophilus ducreyi, Bordetella parapertussis, Bordetella pertussis, Moraxella catarrhalis, Enterobacter spp.. Гиперосмолярная, полимерная мазевая основа препарата обеспечивает движение воды в ране по направлению к мазевой основе. За счет этого уменьшается отек тканей вокруг раны, сорбируется гнойно-некротическое содержимое раны. Вспомогательные вещества мазевой основы способны проводить офлоксацин в ткани, окружающие рану, чем оказывается влияние, как на поверхностную, так и на глубинную раневую инфекцию. Механизм местноанестезирующего действия лидокаина заключается в стабилизации нейрональной мембраны за счет снижения ее проницаемости для ионов натрия, что препятствует возникновению потенциала действия и проведению импульсов по нервным волокнам. Возможен антагонизм с ионами кальция. Подавляет проведение не только болевых импульсов, но и импульсов другой модальности. Быстро гидролизуется в слабощелочной среде тканей и после короткого латентного периода действует в течение 60-90 минут. Анестезирующее действие лидокаина в 2-6 раз сильнее, чем прокаина. При местном применении расширяет сосуды, не оказывает местнораздражающего действия. При воспалении (тканевой ацидоз) анестезирующая активность снижается. Эффективен при всех видах местного обезболивания.
Фармакокинетика.
Офлоксацин и лидокаин практически не поступают в системный кровоток при нанесении мази на здоровую кожу. После нанесения на раневую поверхность, когда барьерные функции кожи нарушены, в системный кровоток поступает около 3 % примененной дозы офлоксацина. Пик концентрации вещества в крови наблюдается через 3-4 часа. Через 6 часов после аппликации мази офлоксацин в крови не определяется.
Показания
Осложненные инфицированные раны в 1-й фазе раневого процесса разной локализации и происхождения: ампутационные культи конечностей, раны после хирургической обработки гнойных очагов, пролежни, трофические язвы, послеоперационные осложненные раны и фистулы, абсцессы и флегмоны челюстно-лицевого участка после их хирургической обработки.
Профилактика и лечение нагноения ожоговых ран.
Гнойно-воспалительные заболевания кожи, вызванные чувствительной к офлоксацину бактериальной микрофлорой.
Противопоказания
Повышенная чувствительность к офлоксацину или к другим производным хинолона; повышенная чувствительность к лидокаину гидрохлориду или к другой местной анестезии амидного типа (например, бупивакаин, этидокаин, мепивакаин и прилокаин); повышенная чувствительность к другим компонентам препарата.
Способ применения и дозы
Препарат Офлокаин-Дарница® применять местно.
Перед нанесением препарата на раневую поверхность, рану желательно промыть асептическим раствором.
После обычной хирургической обработки ран и ожогов мазь наносить непосредственно на раневую поверхность, после чего накладывать стерильную марлевую повязку. Можно сначала нанести мазь на перевязочный материал, а потом наложить его на раневую поверхность.
Тампонами, пропитанными мазью, заполнить полости гнойных ран после их хирургической обработки, а марлевые турунды с мазью вводить в фистульные ходы.
При лечении гнойных ран мазь применять 1 раз в сутки, при лечении ожогов – ежедневно или 2-3 раза в неделю в зависимости от интенсивности гнойных выделений.
Количество мази, которое используется, определяется площадью раневой поверхности (мазь должна покрывать весь пораженный участок) и степенью гнойной экссудации.
Продолжительность лечения определяет врач в зависимости от динамики очищения раны от гнойного экссудата и сроков купирования воспалительного процесса.
При лечении дерматологических заболеваний мазь тонким слоем наносить на пораженные участки кожи 1-2 раза в день или на марлевую повязку с последующей аппликацией. Не следует использовать более чем 20 г мази в течение 12 часов (что соответствует 600 мг лидокаина гидрохлорида).
Побочные реакции
При применении мази могут иметь место такие реакции: зуд, ощущение жжения, высыпания, гиперемия, мокнутие, аллергические реакции, включая дерматит, крапивницу, отек Квинке. Пропиленгликоль, входящий в состав препарата, может вызвать развитие раздражения кожи.
В случае значительной абсорбции лидокаина возможно развитие системных побочных реакций.
Со стороны нервной системы: нервозность, эйфория, спутанность сознания, головокружение, сонливость, шум в ушах, нарушение зрения, рвота, сонливость, судороги, тремор, потеря сознания, угнетение дыхания.
Со стороны сердечно-сосудистой системы: брадикардия, артериальная гипотензия, AV-блокада.
При системном использовании фторхинолонов зарегистрированы случаи удлинения интервала QT на ЭКГ, желудочковой аритмиии пируэтной тахикардии (torsades de pointes) (сообщалось преимущественно у пациентов с известным риском удлинения интервала QT).
Передозировка
О случаях передозировки не сообщалось. При использовании препарата на обширных поверхностях существует вероятность системной абсорбции и токсических эффектов лидокаина.
Симптомами передозировки являются: беспокойство, головокружение, тремор, сонливость, нарушение зрения, бледность, брадикардия, артериальная гипотензия, AV-блокада, коллапс, кома.
При появлении первых симптомов передозировки, использование препарата следует прекратить и обратиться к врачу. Следует проводить мониторинг ЭКГ.
Лечение: симптоматическое.
Меры предосторожности
Если развилась аллергическая реакция, прием препарата, необходимо прекратить. Лидокаин может всасываться через слизистые оболочки, а также через поврежденную кожу.
При использовании на обширных участках раневой поверхности, уровень лидокаина в плазме крови может достигать высоких значений, что способствует развитию серьезных системных побочных эффектов. Пациенты должны быть проинструктированы о необходимости придерживаться рекомендаций по дозированию препарата.
Следует соблюдать особую осторожность при использовании мази у пациентов с эпилепсией, нарушением функции печени, брадикардией, нарушением проводимости сердца, лиц в шоковом состоянии из-за риска развития системных эффектов лидокаина. Риск системной токсичности увеличивается при одновременном использовании других местных анестетиков.
Необходимо проинформировать пациентов, что при использовании лидокаина, входящего в состав мази, может нарушаться чувствительность обработанных участков. По этой причине пациент должен избегать случайного травмирования обрабатываемой области (например, расчесывания, воздействия высоких или низких температур) до полного восстановления чувствительности.
Следует избегать попадания препарата в глаза.
Осторожность следует соблюдать при использовании фторхинолонов, в том числе офлоксацина, у пациентов с известными факторами риска удлинения интервала QT: врожденный синдром удлинения интервала QT; электролитные нарушения, особенно некорректируемая гипокалиемия, гипомагниемия; пожилой возраст; заболевания сердца (например, сердечная недостаточность, инфаркт миокарда, брадикардия); сопутствующим использованием лекарственных средств, которые удлиняют интервала QT (например, антиаритмические лекарственные средства класса IA и III, трициклические антидепрессанты, макролиды, нейролептики).
После системного применения офлоксацина возможно развитие фотосенсибилизации, хотя сообщений о фототоксичности после местного применения не зарегистрировано, нельзя исключить вероятность ее развития. Во время лечения препаратом, следует избегать облучения обрабатываемой поверхности солнечными и искусственными ультрафиолетовыми лучами.
Применение в период беременности или кормления грудью
Препарат не применять в период беременности или кормления грудью.

Способность влиять на скорость реакции при управлении автотранспортом или работе с другими механизмами
Специальных исследований не проводилось. Следует отказаться от управления потенциально опасными механизмами в случае, если препарат применяется на частях тела, которые участвуют в данном виде деятельности.
Взаимодействие с другими лекарственными средствами и другие виды взаимодействий
Абсорбция лидокаина через кожу обычно низкая, однако необходимо с осторожностью применять препарат больным, принимающим противоаритмические препараты I класса (токаинид, мексилетин) или другие местные анестетики, так как возможен риск возникновения совокупных системных эффектов.
Ингибиторы моноаминооксидазы усиливают местноанестезирующее действие лидокаина. Применение лекарственных средств, которые снижают клиренс лидокаина (например, циметидин, пропранолол, верапамил, дилтиазем), в случае его системной абсорбции, может привести к повышению концентрации лидокаина в плазме.
Офлоксацин, как и другие фторхинолоны, следует применять с осторожностью у пациентов, получающих лекарственные средства с известным фактором риска удлинения интервала QT (например, антиаритмические лекарственные средства класса IA и III, трициклические антидепрессанты, макролиды, нейролептики).
Несовместимость
Препарат нежелательно смешивать с другими лекарственными средствами.
Условия хранения
Хранить в недоступном для детей месте в оригинальной упаковке, для защиты от воздействия влаги, при температуре от 8 °C до 15 °C.

По 15 г в тубе алюминиевой с мембраной для медицинских целей; по 1 тубе вместе с инструкцией по медицинскому применению в пачке.
