Фузидовая кислота относится к группе фузидинов, антимикробных соединений, механизм действия которых связан с нарушением синтеза белка в бактериальной клетке.
К фузидовой кислоте чувствительны грамположительные бактерии и грамотрицательные кокки, например Staphylococcus aureus, Staphylococcus epidermidis (включая метициллинрезистентные штаммы), Nocardia asteroides, Corynebacterium minutissimum, Streptococcus pyogenes, Pseudomonas spp., Clostridium spp., Neisseria meningitidis, Neisseria gonorrhoeae.
Неактивен в отношении Escherichia coli, Salmonella spp., Proteus spp. и других грамотрицательных бактерий, а также простейших и грибов.
Фармакокинетика
Проницаемость фузидовой кислоты через интактную кожу низкая. В системный кровоток не попадает.

Кожные инфекции, вызванные бактериями, чувствительными к фузидовой кислоте.

Наружно.
Взрослые и дети с рождения до 18 лет. Наносят на пораженные участки кожи тонким слоем 3–4 раза в день.
При использовании марлевых повязок частота нанесения может быть уменьшена до 1–2 раз в день.
Продолжительность лечения зависит от формы и тяжести заболевания и в среднем составляет 7–14 дней.
Побочные действия

Со стороны кожи и подкожных тканей: нечасто — сыпь (эритематозная, макулопапулезная, пустулезная), раздражение кожи, включая боль, чувство жжения, чувство покалывания на коже, зуд, сухость кожи, контактный дерматит; редко — крапивница, ангионевротический отек, экзема, периорбитальный отек, эритема.
Со стороны иммунной системы: редко — аллергические реакции.
Со стороны органа зрения: редко — раздражение конъюнктивы.
Если любые из указанных в инструкции побочных эффектов усугубляются, или пациент заметил любые другие побочные эффекты, не указанные в инструкции, следует сообщить об этом врачу.

Не следует наносить Фуцидин® на кожу в области глаз, поскольку фузидовая кислота, при попадании в глаза, вызывает раздражение конъюнктивы.
Фуцидин® крем содержит бутилгидроксианизол, цетиловый спирт и калия сорбат, которые могут послужить причиной появления местных аллергических кожных реакций (например контактный дерматит). Бутилгидроксианизол также может послужить причиной появления раздражения глаз и слизистых оболочек.
Фуцидин® мазь содержит ланолин и цетиловый спирт, которые могут послужить причиной появления местных аллергических кожных реакций (например контактный дерматит).
При использовании фузидовой кислоты может развиться бактериальная устойчивость. Как и у других антибиотиков, длительное или периодически повторяющееся использование может увеличить риск развития устойчивости к антибиотикам.

Фармакологическое действие
Антибиотик. Оказывает бактериостатическое или бактерицидное действие. Механизм действия связан с подавлением синтеза белка за счет ингибирования фактора, необходимого для транслокации белковых субъединиц и элонгации пептидной цепи, что приводит в дальнейшем к гибели возбудителя.
Высокоактивен в отношении Staphylococcus spp., особенно Staphylococcus aureus и Staphylococcus epidermidis (включая метициллин-резистентные штаммы), Nocardia asteroides, Clostridium spp.
Менее активен в отношении Streptococcus spp., Enterococcus spp., Neisseria spp., Bacteroides spp., Mycobacterium tuberculosis, Mycobacterium leprae.
Фузидовая кислота активна в отношении некоторых простейших, включая Giardia lamblia, Plasmodium falciparum.
Фармакокинетика
После приема внутрь и в/в введения широко распределяется в организме. Фузидовая кислота накапливается в гное, мокроте, костной ткани, синовиальной жидкости, секвестрах, ожоговых струпьях, абсцессах головного мозга в концентрациях, превышающих МПК для Staphylococcus aureus (0.03-0.16 мкг/мл). Метаболизируется в печени. Выводится с желчью, небольшое количество — с мочой.
При наружном применении фузидовая кислота хорошо проникает как через поврежденную, так и здоровую кожу, при этом системная абсорбция низкая.
При местном применении хорошо проникает во внутриглазную жидкость. Терапевтические концентрации сохраняются в слезной и внутриглазной жидкости не менее 12 ч.
Показания активных веществ препарата
Фуцидин®
Инфекционно-воспалительные заболевания, вызванные преимущественно Staphylococcus spp.: для приема внутрь и в/в введения — абсцесс головного мозга, инфекции костей и суставов, эндокардит, инфекции глаз, септицемия, пневмония, инфекции кожи и мягких тканей, инфекционно-воспалительные осложнения при муковисцидозе, раневая инфекция; для наружного применения — импетиго, карбункулы, фурункулы, инфицированные раны, гидраденит, фолликулит, паронихия, сикоз бороды, эритразма, обыкновенные угри; для местного применения в офтальмологии — конъюнктивит, блефарит, ячмень, дакриоцистит, кератит, инфекции, связанные с удалением инородного тела из конъюнктивы и роговицы.
Режим дозирования
Способ применения и режим дозирования конкретного препарата зависят от его формы выпуска и других факторов. Оптимальный режим дозирования определяет врач. Следует строго соблюдать соответствие используемой лекарственной формы конкретного препарата показаниям к применению и режиму дозирования.
Взрослым внутрь — по 500 мг каждые 8 ч. Детям в возрасте до 1 г — 50 мг/кг/сут в 3 приема, в возрасте 1-5 лет — по 250 мг 3 раза/сут, в возрасте 5-12 лет — по 500 мг 3 раза/сут, старше 12 лет — по 750 мг 3 раза/сут.
Взрослым и детям старше 12 лет внутрь — по 250-500 мг 2-3 раза/сут. Взрослым в/в в виде инфузий — по 500 мг 3 раза/сут. Длительность инфузии — не менее 2 ч.
Детям в возрасте до 12 лет внутрь — 50 мг/кг/сут в 3 приема; в/в — 20 мг/кг/сут, кратность введения — 3 раза/сут.
Максимальная суточная доза для взрослых при в/в введении составляет 2 г.
При наружном применении фузидовую кислоту наносят тонким слоем на пораженные участки кожи 2-3 раза/сут в течение 7 дней. При лечении обыкновенных угрей требуются более длительные курсы терапии.
Местно при заболеваниях глаз применяют 2 раза/сут. Лечение следует продолжать в течение 2 дней после исчезновения клинических симптомов заболевания.
Побочное действие
Со стороны пищеварительной системы: возможны диспептические симптомы, желтуха, нарушение функции печени.
Аллергические реакции: возможна кожная сыпь.
Противопоказания к применению
Повышенная чувствительность к фузидовой кислоте.
Применение при беременности и кормлении грудью
При необходимости применения при беременности и в период лактации следует оценить предполагаемую пользу для матери и потенциальный риск для плода.
B экспериментальных исследованиях установлено, что фузидовая кислота не оказывает тератогенного действия.
Фузидовая кислота проникает через плацентарный барьер. В экспериментах in vitro установлено, что фузидовая кислота замещает билирубин в связи с альбумином. Поэтому следует избегать применения фузидовой кислоты в III триместре беременности из-за возможного развития ядерной желтухи у новорожденных. Этот факт следует учитывать при необходимости применения у новорожденных (в т.ч. недоношенных) детей с желтухой и ацидозом.
Фузидовая кислота выделяется с грудным молоком в очень низких концентрациях.
Применение при нарушениях функции почек
Следует учитывать, что фузидовую кислоту можно применять при нарушениях функции почек и в случаях, когда противопоказано применение других антибиотиков.

Лекарственное взаимодействие
При одновременном применении с колестирамином происходит образование нерастворимых комплексов, что приводит к снижению концентрации фузидовой кислоты в плазме крови.
При одновременном применении фузидовой кислоты с пенициллинами, устойчивыми к действию пенициллиназ, цефалоспоринами, эритромицином, рифампицином, линкомицином наблюдается аддитивное действие или синергизм.
Фармацевтическое взаимодействие
Растворы для инфузий, содержащие фузидовую кислоту, несовместимы с канамицином, гентамицином, ванкомицином, цефалоридином, карбенециллином, растворами аминокислот, кровезамещающими и кальцийсодержащими препаратами. Преципитация фузидовой кислоты может произойти в инфузионных растворах при рН ниже 7.4.

Фузидовая кислота относится к группе фузидинов, антимикробных соединений, механизм действия которых связан с нарушением синтеза белка в бактериальной клетке.
К фузидовой кислоте чувствительны грамположительные бактерии и грамотрицательные кокки, например, Staphylococcus aureus, Staphylococcus epidermidis (включая метициллин-резистентные штаммы), Nocardia asteroides, Corynebacterium minutissimum, Streptococcus pyogenes, Pseudomonas spp, Clostridium spp., Neisseria meningitidis, Neisseria gonorrhoeae, Неактивен в отношении Escherichia coli, Salmonella spp., Proteus spp. и др. грамотрицательных бактерий, а также простейших и грибов.

У взрослых и детей с рождения до 18 лет крем наносят на пораженные участки кожи тонким слоем 3-4 раза в день.
При использовании марлевых повязок частота нанесения может быть уменьшена до 1-2 раз в день.
Продолжительность лечения зависит от формы и тяжести заболевания и в среднем составляет 7-14 дней.

Cipladine ointment is an iodophore with a powerful broad-spectrum germicidal activity against a wide range of bacteria, viruses, fungi, protozoa and spores. Microbicidal; oxidizes cell constituents; iodinates proteins and inactivates them.
Povidone iodine is a kind of iodine disinfectant which directly cause in vivo protein denaturation, precipitation of bacteria, and further resulting in the death of pathogenic microorganisms. Therefore, it is effective in disinfection and sterilization. It can kill viruses, bacteria, spores, fungi, and protozoa with low toxicity to human. Cipladine ointment aqueous solution has strong pharmacological activity against Staphylococcus aureus, Neisseria gonorrhoeae, Pseudomonas aeruginosa, syphilis, hepatitis B virus, HIV, and Trichomonas vaginalis.Povidone iodine gel is a gynecological topical semi-mobile colloidal agent made by povidone iodine and hydrophilic matrix. It is a system for maintaining its sustained release. Owing to the continuous release of free iodine, it can enable the skin and mucous membranes to maintain a certain effective concentration of iodine for killing bacteria. It is mainly used for gynecological vaginal infection. It exerted its effect through being miscible with vaginal secretions and further killing the inside pathogenic microorganisms, and thus blocking the spread of sexually transmitted diseases and invasion, as well as treating other infected vaginal diseases caused by other kinds of bacteria.
Published reports on the in vitro antimicrobial efficacy of iodophors demonstrate that iodophors are bactericidal, mycobactericidal, and virucidal but can require prolonged contact times to kill certain fungi and bacterial spores. Three brands of povidone-iodine solution have demonstrated more rapid kill (seconds to minutes) of S. aureus and M. chelonae at a 1:100 dilution than did the stock solution. The virucidal activity of 75–150 ppm available iodine was demonstrated against seven viruses. Other investigators have questioned the efficacy of iodophors against poliovirus in the presence of organic matter and rotavirus in distilled or tapwater. Manufacturers’ data demonstrate that commercial iodophors are not sporicidal, but they are tuberculocidal, fungicidal, virucidal, and bactericidal at their recommended use-dilution.

How Cipladine ointment works
Cipladine ointment is called iodophore which means povidone acts as a carrier of iodine. Iodine is considered as the active moiety that mediates microbicidal actions. When released from the complex, free iodine (I2) penetrates the cell wall of microorganisms quickly, and the lethal effects are believed to result from disruption of protein and nucleic acid structure and synthesis. While the full mechanism of action is not fully elucidated, iodine is thought to inhibit vital bacterial cellular mechanisms and structures, and oxidizes nucleotides fatty or amino acids in bacterial cell membranes . Additionally, free iodine disrupts the function of the cytosolic enzymes involved in the respiratory chain, causing them to become denatured and deactivated . In vitro evidence suggests that iodine also counteracts inflammation elicited by both pathogens and the host response via multifactorial effects. In hosts, povidone-iodine was demonstrated to modulate the redox potential, inhibit the release of inflammatory mediators such as TNF-α and β-galactosidase, inhibit metalloproteinase production, and potentiate the healing signals from pro-inflammatory cytokines by activation of monocytes, T-lymphocytes, and macrophages, in vitro .

Cipladine ointment dosage
Pre-operative scrubbing & washing: Wet hands with water, then pour 5 ml of solution onto the palm and spread over both the hands, rub the scrub over the entire area for about 2 mintues, add water to develope copious suds, rinse throughly under running water.
Pre- and post-operative skin cleansing: After the skin is shaved and wet with water, apply surgical scrub & rub throughly for minimum 2 mintues, rinse of with the sterile gauze saturated with water.
Side Effects
Local skin irritation and sensitivity. Application to large areas of denuded skin may produce systemic effects due to iodine absorption.The application of povidone iodine to large wounds or severe burns may producesystemic adverse effectssuch as metabolic acidosis, hypernatraemia and impairment of renal function.
